Stroke Risk Among Elderly Users of Haloperidol and Typical Antipsychotics Versus Atypical Antipsychotics: A Real-World Study From a US Health Insurance Claims Database
Antipsychotics are approved and prescribed to treat various conditions such as schizophrenia, mania, major depressive disorder, agitation, delusional disorder, psychosis, and Tourette's syndrome.1 The haloperidol prescribing information2 in the United States (US) does not warn about the risk of stroke but has a black box warning for an increased risk of death among elderly patients with dementia-related psychosis treated with antipsychotic drugs. Nonetheless, antipsychotics are used for indications such as neuropsychiatric symptoms of dementia, in the presence or absence of psychosis; such use is not approved by the Food a...

Can cardiovascular risk management be improved by shared care with general practice to prevent cognitive decline following stroke/TIA? A feasibility randomised controlled trial (SERVED memory)
Cognitive impairment and dementia following cerebrovascular disease are increasingly common in the UK. One potential strategy to prevent post-stroke cognitive decline is multimodal vascular risk factor managem...

Stress, Burden, and Well-Being in Dementia and Non-Dementia Caregivers: Insights from the Caregiving Transitions Study.
DISCUSSION AND IMPLICATIONS: Dementia and non-dementia caregivers showed relatively few differences in indicators of overall well-being in this population-based sample, perhaps because both groups of caregivers in this study were providing substantial care. Dementia caregivers may require special assistance with dementia-specific problems such as behavioral problems. Clinical interventions and policy changes targeting highly burdened caregivers are needed to support them in allowing their care recipients to age in place at home. Cognitive health expectancies of cardiovascular risk factors for cognitive decline and dementia
ConclusionsHaving a cardiovascular condition is associated with a lower CIFLE and higher proportion of life lived with cognitive impairment. However, the outcomes vary depending on the type of cardiovascular condition. Reducing incidence of stroke and minimising exposure to multiple cardiovascular risk factors may be beneficial in helping to improve population estimates of cognitive health expectancy.

Spatiality of care : Spatial challenges in informal care of people with dementia qualitatively researched.
Abstract With respect to the diversity of age and aging, dementia poses a key challenge in our present long living society. In this respect dementia is not only a challenge for those who are diagnosed with dementia but also for their long-term caregivers. The majority of people with dementia receive care from close relatives, such as life partners in their own four walls; however, what is often a stroke of luck for the person with dementia, often means a lot of strain and spatial restrictions for the caring relative. With a praxeologically guided perspective on nursing, this article focuses on the subjective e...

Interactions Between Acute Infarcts and Cerebrovascular Pathology Predict Poststroke Dementia
Conclusions: The risk of PSD varies depending on the presence of chronic cerebrovascular pathologies and type of acute infarcts. Clinical implications support a precision medicine approach for stratifying those at highest risk of PSD.

Neurobiological Targets of Apathy Can Guide Treatment Development
Apathy is a motivational disturbance leading to reduced goal-oriented behavior. It afflicts a large percentage of patients suffering for Alzheimer's disease but it is also common in many neuropsychiatric disorders, including Parkinson's disease, frontotemporal dementia, stroke, and idiopathic depression. Apathy is disabling and adds to caregiver burden and distress. In an authoritative review, van Dyke et al1 summarize evidence indicating that compromise of the prefrontal cortex by the neuropathological changes of Alzheimer's disease lead to the lack of motivation characterizing apathy.

Neurogeriatrics-a  vision for improved care and research for geriatric patients with predominating neurological disabilities.
This article discusses how neurological know-how could be integrated in academic geriatric medicine to improve care of neurogeriatric patients, to foster neurogeriatric research and training concepts and to provide innovative care concepts for geriatric patients with predominant neurological conditions and disabilities. More than a quarter century of the most prescribed sleeping pill: Systematic review of zolpidem use by older adults.
Abstract Zolpidem is widely used to treat insomnia of older adults despite that few randomized controlled studies were conducted in this group. We systematically reviewed the relevant literature on efficacy/effectiveness and safety of zolpidem use by elderly individuals in relevant databases completed with a manual search of key journals. Studies were required to include individuals aged ≥60 years under intervention with zolpidem compared to placebo or other hypnosedatives. Outcomes were either objectively- or subjectively-assessed improvements in specific sleep parameters and safety for clinical use. The 31 r...

Baseline frailty status and outcomes important for shared decision-making in older adults receiving transcatheter aortic valve implantation, a prospective observational study.
CONCLUSIONS: TAVI patients had improvement in symptoms and maintenance of activity of daily living at 6 months. They had low mortality and most patients lived in their own home 2 years after TAVI. Complications like death, stroke, and endocarditis occurred. Some patients had cognitive impairment before the procedure which might influence decision-making. Our findings may be used to develop pre-TAVI decision aids. PMID: 32193850 [PubMed - as supplied by publisher]
