[Facets of moral distress in nusing homes : A qualitative study with examined registered nurses].
Abstract Although the nursing home setting has some characteristic features that are relevant with respect to the development of moral distress (MD), it has so far been studied primarily in the acute care setting. The underlying classical definition of MD is increasingly criticized and broadening of this definition is encouraged to include stress due to moral uncertainty. Hence, the present qualitative study explored how nursing personnel in nursing homes experience MD. Guided interviews were conducted with 21 nurses from 5 nursing homes. The fully transcribed interviews were analyzed using Mayring's qualitat...

A  pilot observational study to analyze (in)activity and reasons for sedentary behavior of cognitively impaired geriatric acute inpatients.
CONCLUSION: Time spent without company, lacking meaningful activities and continuous bedridden periods due to missing demands to leave the room might have led to time spent inactive and alone. These seem to be strong predictors for sedentariness. Routines of acute care should be reorganized to increase physical activity and thereby reduce sedentary behavior of this patient group. Melatonin improves the osseointegration of hydroxyapatite-coated titanium implants in senile female rats.
Abstract The aim of this study was to confirm the effect of the systemic administration of melatonin on hydroxyapatite-coated titanium (HA-Ti) implants in senile osteopenic rats. For this study 24-month-old female Sprague-Dawley rats were used. The animals were randomly divided into two groups: a control group and a melatonin group and the bilateral femurs of all the rats received HA-Ti implants. Animals in the melatonin group received treatment with melatonin (30 mg/kg day). After a 12-week healing period, rats in the melatonin group revealed improved osseointegration compared to the control group, with the bon...

Reliability of the questionnaire was calculated using Cronbach's alpha (= 0.762). [Wife against advance directive: a case-inspired legal and ethical clarification].
b G Abstract Advance directives and durable powers of attorney are two increasingly used tools for patients' precedent autonomy; however, their combined use, as recommended by major institutions in Germany, might in reality result in various discrepancies between the wording and interpretation of the directive and the surrogate's understanding of the patient's relevant will. In one of the possible conflict constellations, the surrogate wants to overrule a relevant and unambiguous advance directive by reference to a privileged access to the patient's "real" treatment preferences or the presumed will ...

l;ger D Abstract With increasing longevity, questions about the psychosocial conditions for extraordinary life spans come to the fore. From the field of psychology the concept of resilience is used to describe special capabilities to resist as a personal competence to maintain or regain certain levels of functioning. As such, resilience is seen as a relatively stable personality trait that, however, may forfeit its relevance in old age.The present article, in contrast to the usual phases of life view, adopts a sociological perspective on the entire life span. The rational choice approach is utilized...

[Dignity, freedom, embodiment : Ethical categories concerning the use of freedom-depriving measures in people with dementia in acute care settings].
Abstract The use of freedom-depriving measures (physical and medicinal restraints) in people with cognitive impairment or dementia in clinical care settings is of ongoing importance. At the same time, these coercive measures are not only heavily debated but also in most cases ethically questionable from the perspective of the ethics of human dignity. Usually, the ethical evaluation of freedom-depriving measures follows classical paradigms of medical ethics, such as the Principles of Biomedical Ethics by Beauchamp and Childress. To enrich the debate at this point, the ethical category of embodiment ("Leiblichk...

[Heart failure in old age : Current recommendations in view of the revised national treatment guidelines on chronic heart failure].
Abstract Clinical symptoms of heart failure in older patients can be similar to those of other diseases in geriatrics. Therefore, the diagnosis should be confirmed by echocardiography. The treatment of older patients with heart failure with reduced left ventricular ejection fraction (HFrEF) should be carried out in accordance with the guidelines with regular control of volume status, renal function and serum electrolytes. Due to the lack of effective treatment of heart failure with preserved ejection faction (HFpEF) blood pressure control as a measure of prevention is important. Patients can benefit from the ...

Abstract Malignant wounds arise either primary or secondary in the context of a malignant transformation of already existing wounds. A plethora of skin tumors, such as basal cell carcinoma, squamous cell carcinoma, melanoma, lymphoma as well as cutaneous metastases of other malignancies can ulcerate and be the cause of malignant wounds. Ulcerating tumors or metastases of the skin can however mimic chronic wounds from other causes and remain unrecognized over a longer period. In patients with chronic ulcerations, the correct and timely diagnosis is paramount. Based on this, the stage and disease-orie...
