Challenges in the management of hypertension in Asia
Hypertension is common in Asian populations and is a major cause of cardiovascular diseases. Even a small reduction in blood pressure can lead to a substantial decrease in the risk of stroke. The prevalence of hypertension appears to be increasing in many countries, partly because of ageing of the population, but also because of increasing obesity. While much effort has gone into promoting the awareness of hypertension, the rates of detection, treatment, and control remain low. As hypertension is very prevalent in the general population, it makes sense to adopt population approaches towards the detection and treatment of h...

Pneumonia: When Antibiotics Are Not the Answer
We present the case of an 88-year-old bedridden man, with hypertension, multiple myeloma, coronary artery disease, ischemic stroke, seizures, dysphagia, and constipation under treatment with mineral oil. He was brought to the emergency department after 3 days of cough, dysphagia, and dyspnea, following a recent hospitalization, due to a health care–associated pneumonia. A chest computed tomography scan obtained showed bilateral multifocal opacities with consolidates described as Hounsfield units of −62, consistent with fat. A diagnosis of lipoid pneumonia was done, and mineral oil was discontinued. Given the patient’...

What 'Empire' Got Right (And Wrong) About Music Therapy
Perhaps one of the most stirring and sympathetic characters in Fox’s hit show “Empire" is Andre, who suffers from Bipolar disorder. In case you’ve been living under a rock for the past three months and haven’t watched the hottest TV show of 2015, here’s a quick recap of Andre’s situation: the oldest son of a music conglomerate CEO vies for power over the company he helped build, but between all the pressure (and betrayal, and violence, and lack of love and support), as well as his attempts to keep a lid on his emotions, Andre eventually flushes his meds down the toilet, precipitating a mental breakdown and entr...

9 Science-Backed Reasons Why You Should Go to Bed Early
Show of hands if you've ever stayed up way too late texting with friends, catching up on TV shows, or scrolling aimlessly through Pinterest or Reddit. Yep, plenty of us are guilty as charged -- and as a result, we end up feeling exhausted in the morning. That most Americans are falling short on getting the recommended eight hours of sleep per night is nothing new. Clearly though, not everyone who fails to log enough shut eye actually has trouble sleeping. Plenty of us are just staying up too late or putting off bedtime in favor of other activities. But over time, staying awake into the wee hours can come with a hefty cos...

Respiratory and otolaryngologic manifestations of giant cell arteritis.
CONCLUSIONS: It is important to recognize these atypical presentations because they may be the sole initial manifestation of the disease. Early suspicion and confirmation of the diagnosis of GCA can help to prevent more catastrophic consequences of unrecognized disease, including stroke and blindness. Could pneumonia be causing your cough?
IT STARTS with a sore throat, runny nose, sneezing and a cough, just like a common winter cold. But pneumonia is expected to claim the lives of 50,000 adults this year. It is only behind heart attack, stroke and cancer as the UK's major killer.

European headache federation consensus on technical investigation for primary headache disorders
Abstract The diagnosis of primary headache disorders is clinical and based on the diagnostic criteria of the International Headache Society (ICHD-3-beta). However several brain conditions may mimic primary headache disorders and laboratory investigation may be needed. This necessity occurs when the treating physician doubts for the primary origin of headache. Features that represent a warning for a possible underlying disorder causing the headache are new onset headache, change in previously stable headache pattern, headache that abruptly reaches the peak level, headache that changes with posture, headache awaken...

Comparison of the Efficacy and Safety of Different ACE Inhibitors in Patients With Chronic Heart Failure: A PRISMA-Compliant Network Meta-Analysis
Abstract: Heart failure is a public health problem and a great economic burden for patients and healthcare systems. Suppression of the renin–angiotensin system (RAS) by angiotensin-converting enzyme (ACE)-inhibitors remains the mainstay of treatment for heart failure. However, the abundance of ACE inhibitors makes it difficult for doctors to choose. We performed this network meta-analysis of ACEIs in patients with heart failure in order to address this area of uncertainty. We searched PubMed, Embase, CENTRAL, and Medline. Any randomized controlled trial evaluating the efficacy and safety of captopril, enalapril, lisin...
