Risk Factors for Recurrence of Nasal Polyps after Endoscopic Sinus Surgery in Patients with Allergic Chronic Rhino Sinusitis
AbstractRhino sinusitis, is a common inflammatory disease that affects the nasal cavity and paranasal sinuses in millions of individuals. Chronic sinusitis patients complain of a combination of nasal obstruction, rhinorrhea, post nasal drip and facial pain. To identify the risk factors of the recurrence of nasal polyps in chronic rhino sinusitis patients after endoscopic sinus surgery. After a review of 60 patients with chronic rhino sinusitis with failed medical treatment requiring FESS presenting in Kasr Alainy Hospital in Cairo University and October 6 University Hospital from May 2022 to March 2023. All patients were s...

Characteristics of chronic rhinosinusitis phenotypes in patients undergoing functional endoscopic sinus surgery: an observational cohort retrospective study
This study analyzed the characteristics of chronic rhinosinusitis patients with and without nasal polyps failing conservative treatment and undergoing functional endoscopic sinus surgery as part of their treatment for chronic inflammatory sinonasal disease. The aim of this retrospective single-institution cohort study conducted at a university hospital tertiary referral center was to evaluate the characteristics of patients with both disease phenotypes in whom conservative treatment was unsuccessful. Patients who were surgically treated with functional endoscopic sinus surgery performed by two rhinology surgeons during a o...

Multidisciplinary checklist approach for the 'severe phenotype: Asthma with Nasal Polyps
Asthma with chronic rhinosinusitis with nasal polyps (CRSwNP) associates with higher burden and more difficult treatment.Aim: To propose an evidence-based data checklist for a coordinated management of these patients.A multidisciplinary team of ENT physicians, allergologists, pneumologists and hospital pharmacist aimed to develop a checklist for the assessment of patients, based on the diagnostic definition, and the control and severity criteria establish in guidelines such GINA, ARIA, EPOS, and international consensus on biologic therapies for CRSwNP and Asthma from EUFOREA and from EAACI, and a multidisciplinary consensu...

Confirmation of endotypes based on levels of eosinophil, mast cell and basophil microparticles in nasal lavage fluids in chronic rhinosinusitis with nasal polyps using two Northwestern Memorial Hospital cohorts
Microparticles (MPs) are extracellular vesicles that are released by shedding from plasma membrane of activated cells and new biomarkers to evaluate status of cell activation. MP profilings of activated eosinophils, mast cells and basophils in nasal mucosa in chronic rhinosinusitis with nasal polyps (CRSwNP) remain unclear.

What Are Common Gastrointestinal Polyposes in Children?
Discussion Bloody stools or blood in the diaper is a relatively common complaint in general pediatrics. For most patients it is often a minor concern. Commonly it is a transient problem (e.g. rectal fissure caused by constipation or trauma) or often not blood (e.g. urate crystals in the diaper, food, menses, etc.). Real blood does occur with an identifiable cause such as long distance running or heavy exercise, or milk protein allergy/sensitivity that improves with a milk-restricted diet. Many more serious causes are associated with heavier or more recalcitrant bleeding, increased defecation, abdominal pain, poor eating an...

Are objective 'findings' the same as subjective 'severity'? A study of the relationship between computed tomography findings and subjective severity in preoperative CRSwNP patients.
Authors: Lu H, Liu Z, Hu L, Feng X, Hu L, Gu Y, Li H, Li H, Wang D Abstract In pre-surgical patients with chronic rhinosinusitis with nasal polyps (CRSwNPs), positive findings on computed tomography (CT) scans and an exacerbation of symptoms are two possible factors that indicate surgery. However, the relationship between these factors remains unclear. Confirmed CRSwNP patients ready for sinus surgery were consecutively enrolled. The Sino-Nasal Outcome Test 22 (SNOT-22) and visual analog scale (VAS) scoring were completed by these patients, and scores were subjected to factor analysis using a principal component an...

Loss-of-Function in SMAD4 Might Not Be Critical for Human Natural Killer Cell Responsiveness to TGF- β
This study was carried out in accordance with approval of the Melbourne Health and Walter and Eliza Hall Institute of Medical Research's Human Research Ethics Committee (approval number: 2013.081). All subjects gave written informed consent for participation and publication. Results and Discussion TGF-β signaling in NK cells is associated with: phosphorylation in SMAD2 and 3, inhibition of IL-15-induced metabolism/proliferation, simultaneous downregulation of CD44, CD49e, and Eomes, and upregulation of CD16 and CD49a expression (7, 10). SMAD family member 4 (SMAD4) belongs to the SMAD family of transcript...

LPS may enhance expression and release of HMGB1 in human nasal epithelial cells in vitro.
The objective of the present study was to investigate the relocation of HMGB1 in LPS-induced human nasal epithelial cells in vitro. We obtained epithelial cells of nasal polyps from 10 patients requiring surgery for sinusitis at the ENT Department of the Chinese PLA General Hospital. The primary cultured human nasal epithelial (HNE) cells were stimulated with LPS. The expression and translocation of HMGB1 in intracellular and culture supernatants were determined using Western blot and immunofluorescence assay. HMGB1 protein was released in a time-dependent fashion in culture supernatants: in fact, expression of HMGB1 prote...

SNOT-22 Validation in Greek Patients
Background: Chronic rhinosinusitis (CRS) with or without nasal polyps is a frequent and significant health problem. The 22-item Sinonasal Outcome Test (SNOT-22) is a valid, disease-specific health status instrument translated into several languages. The translation into Greek has been considered essential for the individual assessment of the patients' symptoms and a reliable tool for quality of life evaluation. Methods: Our study included 40 patients with CRS without nasal polyps and 40 healthy individuals as control group recruited from the ENT Allergy and Endoscopy Clinic of Chania General Hospital. Assessment included f...

How Much Cetirizine Can Be Used?
Discussion Antihistamine medications have been available for over 70 years. The original H1-antihistamines, while effective in treated allergic rhinitis, urticaria and other allergic problems, easily penetrated the blood-brain barrier and caused somnolence. The newer, second generation H1-antihistamines have much less somnolent side effects because of poor penetrance. Fexofenadine (Allegra®), desloratadine (Claritin®) and levocetirizine (Zyrtec®) are commonly used H1-antihistamines in the US today. Previously Astemazole (Hismanal®) and Terfanadine (Seldane®) were used but were withdrawn because of prob...
