Are oestrogens and genetic predisposition etiologic factors in the development of clear cell carcinoma of the peritoneum?
Abstract: A literature search was carried out for clinical observations that could explain the possible aetiology of primary peritoneal clear cell carcinoma (CCC) including diagnostic dilemmas, various theories of origin, oestrogen dependence and genetic association. It was found to be an extremely rare tumour (CCC) arising in the peritoneum and is often misdiagnosed as mesothelioma or serous carcinoma or metastatic adenocarcinoma due to overlapping morphological features. The awareness of such dilemmas is important even before making a diagnosis. Clinicopathological features and immunohistochemical studies like WT1, CK20 ...

Surgical concepts in esophageal cancer
SummaryDespite neoadjuvant treatment being available for esophageal cancer, surgery remains the cornerstone of treatment. The aim of this article is to give a clear and simple overview of current issues in the available surgical strategies relating to locally limited and advanced disease, including the following: clinical staging, preoperative general condition and comorbidities, surgical strategy, surgical approaches, postoperative complications and the role of surgery in advanced disease. Based on a literature search and our personal professional experience to date, enhanced surgical treatment protocols for the treatment...

Systematic Search for Present and Potential Portals of Entry for Infective Endocarditis
ConclusionsOur search for the POEs of present IEs was often successful, as was searching for an oral or dental or a gastrointestinal POE of a new IE episode. We advise the systematic performance of stomatologic examinations in patients with IE and performance of colonoscopy in patients ≥50 years of age or at high risk for colorectal cancer.

Port site metastases after robot‐assisted surgery: a systematic review
ConclusionsPort‐site metastasis is an extremely rare complication of robotic surgery. No safe conclusion can be drawn, but the aim of this study was to raise doctors' suspicion levels to such a rare new entity.

Diagnosis and treatment of Barrett's oesophagus
Introduction Barrett's oesophagus (BO) is a common premalignant condition, which carries a risk of progression to oesophageal adenocarcinoma. Recent advances include quantifying the risk of neoplasia progression, novel diagnostic tools and development of new endoscopic techniques to treat early Barrett's cancer. Sources of data A selective search was performed on recent advances in BO and this was supplemented with guidelines from the American and British Society of Gastroenterology. Areas of agreement All cases of dysplasia should be confirmed by a second expert histopathologist. Endoscopic therapy is the preferred opti...

Republished: Symptomatic reflux disease: the present, the past and the future
The worldwide incidence of GORD and its complications is increasing along with the exponentially increasing problem of obesity. Of particular concern is the relationship between central adiposity and GORD complications, including oesophageal adenocarcinoma. Driven by progressive insight into the epidemiology and pathophysiology of GORD, the earlier belief that increased gastroesophageal reflux mainly results from one dominant mechanism has been replaced by acceptance that GORD is multifactorial. Instigating factors, such as obesity, age, genetics, pregnancy and trauma may all contribute to mechanical impairment of the oeso...

Health state utility values associated with advanced gastric, oesophageal, or gastro-oesophageal junction adenocarcinoma: a systematic review.
CONCLUSIONS: Limited data estimate HSUVs in patients with advanced GC, OC, or GEJ cancer. Utilities for advanced GC alone and advanced OC alone were reported in only two publications for each cancer type. No publications considered advanced GEJ utilities alone, and four publications considered utilities for a mixed population of gastrointestinal cancer types. Comparisons are confounded by heterogeneity across the identified publications. Further research into HSUVs associated with advanced GC and OC is required to improve the evidence available for use in CUAs. PMID: 26212479 [PubMed - in process]

Expanding the search for significant EGFR mutations in NSCLC outside of the tyrosine kinase domain with next-generation sequencing
AbstractWhile conventional organization of EGFR mutations in non-small cell lung cancer (NSCLC) includes classic lesions sensitive to tyrosine kinase inhibitors (TKI) and variants localized to the tyrosine kinase domain (TKD) in exons 18 –21, next-generation sequencing (NGS) raises the prospect of identifying clinically relevant variants in extra-TKD regulatory regions. NSCLC patients at our institution who received tumor profiling with NGS from 2013 to 2015 were identified. EGFR mutations were arranged based upon their distributi on relative to the TKD. In silico analysis was performed to predict non-synonymous single n...

Endometriosis with an aberrant immunophenotype: Challenging differential diagnosis of glandular lesions in the pelvic lymph nodes.
CONCLUSIONS: This type of aberrant of the endometriotic gland immunophenotype has never been presented in the scientific literature before. This finding plays a significant role from the pathology standpoint and, perhaps more importantly, from the clinical standpoint. An asymptomatic patient with a correct diagnosis of lymph node endometriosis did not undergo excessive treatment for false positive diagnosis of metastatic cervical adenocarcinoma.
