Anticoagulation prescribing patterns in patients with cancer
The objective of this retrospective cohort was to describe real-world anticoagulation prescribing patterns in cancer patients at a large academic medical center between January 1, 2013 and October 31, 2016. We sought to assess the safety, tolerability, and efficacy of DOACs in patients with cancer for either VTE and/or AF. Patient demographic, clinical characteristics, as well as bleeding and thrombotic events were collected. Anticoagulant Agents for Atrial Fibrillation in Cancer Patients
In a recent article in the journal, Patell et al1 found CHADS2 and CHA2DS2VASC to predict risk of ischemic stroke in cancer patients with baseline atrial fibrillation (AF). Although only 36% of patients received anticoagulant therapy, the risk of stroke generally appeared to be lower than in cancer-free individuals2,3. Given the size and statistical rigor of the analysis, and the perplexing paucity of published data regarding the management of coexisting AF and cancer, the study by Patell et al1 is of outstanding worth.

Efficacy and Safety of Rivaroxaban versus Warfarin in Patients with Nonvalvular Atrial Fibrillation and a History of Cancer: Observations from ROCKET AF.
Conclusion: In ROCKET AF, a history of cancer was associated with a higher risk of bleeding and non-cardiovascular death, but not ischemic events. The relative efficacy and safety of rivaroxaban compared with warfarin were not significantly different in patients with and without a history of cancer. The results of this study are exploratory and should be taken in context of the study population, which may not be generalizable to those with advanced malignancy. Further investigation is needed to understand optimal anticoagulation strategies in patients with AF and cancer. PMID: 30219887 [PubMed - as supplied by publisher]

CONCLUSION:  Utilizing FM concentrations to assess hyper-coagulable patients seems promising; however, there are limitations including variations in FM cut-off values, the effect of patient medications and the timing of FM measurement relative to an acute event. Comparative effectiveness of rivaroxaban versus a vitamin K antagonist in patients with renal impairment treated for non-valvular atrial fibrillation in Germany — A retrospective cohort study
ConclusionThe prescription of rivaroxaban in patients with NVAF and renal impairment was associated with a lower incidence of ischaemic stroke and intracranial haemorrhage versus phenprocoumon in patients without evidence of cancer.

Direct oral Xa inhibitors versus warfarin in patients with cancer and atrial fibrillation: a meta-analysis
Conclusion In patients with cancer and atrial fibrillation, direct oral Xa inhibitors have a similar efficacy and may be safer compared with warfarin. These results are consistent both in patients with active cancer and history of cancer.

The anticoagulation dilemma and future treatment avenues in patients with breast cancer and atrial fibrillation
Atrial fibrillation (AF) is the most common arrhythmia, with a substantial rise in global incidence and prevalence. Ischemic stroke is a frequent complication of AF, since AF perfectly fulfills Virchow's triad of blood stasis, vascular damage and hypercoagulation, making oral anticoagulation (OAC) obligatory for stroke prevention. Direct oral anticoagulants (DOAC), such as dabigatran, which inhibits thrombin, and apixaban, edoxaban and rivaroxaban, which block the activated coagulation factor X (FXa), have some advantages and are largely replacing coumarin-based OAC.
