Medtronic wins expanded PMA for CoreValve
The FDA cleared expanded indications for Medtronic‘s (NYSE:MDT) CoreValve TAVR devices, now cleared for use in patients with end-stage renal disease and those with low-grade, low-flow aortic stenosis. Medtronic won approval from the FDA to modify the CoreValve’s labeling in November which included both its CoreValve and CoreValve Evolut R systems. The changes to the labeling were approved based on Medtronic’s CoreValve U.S. expanded use study of high and very-high-risk subjects at need for aortic valve replacements, according to an email FierceMedicalDevices received from a Medtronic spokesperson. Data from theÂ...

Extracorporeal treatment for digoxin toxicity: just say no
3 out of 5 stars Extracorporeal treatment for digoxin poisoning: systematic review and recommendations from the EXTRIP Workgroup. Mowry JB et al. Clin Toxicol 2016 Feb;54:103-14. Abstract “Forced diuresis, and hemodialysis are ineffective in enhancing the elimination of digoxin because of its large volume of distribution (4-10 L/kg), which makes it relatively inaccessible to these techniques.” Goldfrank’s Toxicologic Emergencies (Tenth Edition, 2015) That one sentence from the latest edition of Goldfrank’s tells you all you need to know about the use of extracorporeal treatment (ECTR)  in digoxin...

Review of synthetic cannabinoids out-of-date as soon as it’s published
3 out of 5 stars A systematic review of adverse events arising from the use of synthetic cannabinoids and their associated treatment. Tait RJ et al. Clin Toxicol 2016 Jan;54:1-13. Abstract This paper review clinical literature related to use of synthetic cannabinoids (SCs) through the end of 2015. Although it seems comprehensive, and cites 108 references, it is of course already at least 18 months out-of-date. The number of cases involving exposure to SC surged during 2015. For example, although the authors of this paper identified 22 deaths associated with SCs, according to CDC data in the first 6 months of 2015 alone ...

Teach Providers to Appreciate the Small Parts of the Job
When the public thinks or hears about firefighting, what typically comes to mind are all the big jobs: multi-alarm fires, extrications, hazardous materials mitigations, collapsed structure and confined space rescues—the kinds of calls we worry about but also somewhat ironically look forward to. This all looks a little different on the medical side. Although many of the things we encounter in suppression can certainly be dramatic and critically important, there are many more opportunities to save or change the course of a life on the EMS end of things. That’s not to belittle or to trivialize the importance of fire suppr...

An In Vitro Murine Model of Vascular Smooth Muscle Cell Mineralization
Vascular calcification (VC) is seen ubiquitously in aging blood vessels and prematurely in disease states like renal failure. It is thought to be driven by a number of systemic and local factors that lead to extra-osseous deposition of mineral in the vascular wall and valves as a common endpoint. The response of resident vascular smooth muscle cell to these dystrophic signals appears to be important in this process. Whilst in vivo models allow the observation of global changes in a pro-calcific environment, identifying the specific cells and mechanisms involved has been largely garnered from in vitro experiments, which pro...

Reduction of contrast medium volume and radiation dose in CTA scans
Researchers have shown that a CTA scan (a scan of the arteries) can be performed with a contrast medium volume reduction of up to 75% and up to 50% reduction in radiation dose. CT scans using contrast medium are the third most common cause of acute renal failure and renal insufficiency as a result of hospital treatments. (Source: ScienceDaily Headlines)

Quantitation of Parathyroid Hormone in Serum or Plasma by Liquid Chromatography-Tandem Mass Spectrometry
Parathyroid hormone (PTH), an 84 amino acid peptide hormone, is an important regulator of calcium homeostasis. Quantitation of PTH in serum is useful for the diagnosis of primary hyperparathyroidism, hypoparathyroidism, and for monitoring osteodystrophy in patients with renal failure. The biological activity of PTH arises from binding of PTH (N terminus) to its target receptor (D’Amour et al., Kidney Int 68: 998–1007, 2005). Several C-terminal and N-terminal fragments circulate in normal subjects. Recent studies have demonstrated that accurate quantitation of PTH fragments may be of clinical value. In this chap...
