36-Year-Old Woman With Progressive Dyspnea, Cough, and Chills
A 36-year-old woman presented to the hospital for evaluation of progressively worsening dyspnea on exertion. She had a pertinent medical history of anxiety, polycystic ovary syndrome, and rheumatoid arthritis work-up with her primary care provider. Her home medications included escitalopram, gabapentin, glucosamine-chondroitin, loratadine, multivitamin, acetaminophen, and ibuprofen. She reported 2 weeks of progressively worsening dyspnea on exertion with associated nonproductive cough, chills, sore throat, and palpitations. (Source: Mayo Clinic Proceedings)

Adverse drug events associated with nortriptyline compared with paroxetine and alternative medications in an older adult population: a retrospective cohort study in Southern California
Conclusions Compared with therapeutic alternatives, nortriptyline was found to represent a lower risk for falls, fractures and syncope, versus comparator medications, except for a few instances that had equivalent risk. The risk for these adverse events from paroxetine was comparable to the alternative medications. (Source: BMJ Open)

Drug-induced QT prolongation and torsade de pointes: a real-world pharmacovigilance study using the FDA Adverse Event Reporting System database
Conclusion: Our study provided a real-world overview of QT prolongation and TdP to drugs, and the presentation of the potential culprit-drug list, the proportion of reports, the detection results of ADR signals, and the distribution characteristics of ADR signals may help understand the safety profile of drugs and optimize clinical practice. Saikosaponin-d alleviates depression by promoting NLRP3 ubiquitination and inhibiting inflammasome activation
This study's objective was to ascertain how SSd may reduce depression in depressed mice subjected to chronic unpredictable animal stress (CUMS) and to investigate the mechanisms underlying these effects. Models of CUMS depression were established and different groups were treated with SSd and escitalopram. After the last day of administration of the treatment, behavioral tests were performed. ELISA was used to measure the expression of IL-1β, TNF-α, and IL-18, and western blot was used to measure the presence of proteins associated with NLRP3. Hippocampal neuronal damage was observed using Nissl staining, and NLRP3 ubiqu...

Effects of electroacupuncture at Baihui (GV20) and Yintang (GV29) on endoplasmic reticulum stress in depressive rats caused by chronic unpredictable mild stress
ConclusionEA at Baihui (GV20) and Yintang (GV29) can significantly mitigate behavioral deficits of rats with CUMS-induced depression. The mechanism may be related to adjusting the ERS and inhibiting cell apoptosis. (Source: Journal of Acupuncture and Tuina Science)

Pharmacological treatments in panic disorder in adults: a network meta-analysis
CONCLUSIONS: In terms of efficacy, SSRIs, SNRIs (venlafaxine), TCAs, MAOIs and BDZs may be effective, with little difference between classes. However, it is important to note that the reliability of these findings may be limited due to the overall low quality of the studies, with all having unclear or high risk of bias across multiple domains. Within classes, some differences emerged. For example, amongst the SSRIs paroxetine and fluoxetine seem to have stronger evidence of efficacy than sertraline. Benzodiazepines appear to have a small but significant advantage in terms of tolerability (incidence of dropouts) over other ...

Machine Learning Prediction of Quality of Life Improvement During Antidepressant Treatment of Patients With Major Depressive Disorder: A STAR*D and CAN-BIND-1 Report
Conclusions: Machine learning can predict normative QoL after antidepressant treatment with similar performance to that of prior work predicting depressive symptom response and remission. These results suggest that QoL outcomes in MDD patients can be predicted with simple patient-rated measures and provide a foundation to further improve performance and demonstrate clinical utility.Trial Registration: ClinicalTrials.gov identifiers NCT00021528 and NCT01655706.PMID:37967350 | DOI:10.4088/JCP.23m14864 (Source: Journal of Clinical Psychiatry)
