External Urethral Sphincter Botulinum Toxin Injection to Treat Pseudodysynergia in Patients With Cerebral Palsy
To evaluate urinary outcomes following cystoscopic external urinary sphincter onabotulinumtoxinA (BTX) injections in patients with cerebral palsy (CP). Adults with CP can suffer from bladder outlet obstruction and urinary retention due to a spastic external urethral sphincter ( “pseudodysynergia”). We have used BTX injections into the sphincter to relieve the obstruction and allow patients to maintain spontaneous voiding rather than intermittent catheterization. (Source: Urology)

Editorial Commentary on: Improving Racial Disparities in Prostate Cancer
Racial disparities in prostate cancer treatment and their subsequent impact on outcomes including mortality have been well described. In addition to lower screening rates, inequities in guideline-concordant treatment, and lower representation in clinical trials,1 there are also disparities in health-related quality of life related to prostate cancer treatment in Black men,2,3 of which androgen-deprivation therapy (ADT)-associated depression is a major component. Racial disparities in prostate cancer treatment and their subsequent impact on outcomes including mortality have been well described. In addition to lower screening rates, inequities in guideline-concordant treatment, and lower representation in clinical trials1, there are also disparities in health-related quality of life (HRQoL) related to prostate cancer treatment in Black men2,3, of which androgen-deprivation therapy (ADT)-associated depression is a major component. Editorial comment
In addition to standard peri-operative prophylaxis, both the American Urological Association and the Infectious Diseases Society of America recommend screening for asymptomatic bacteriuria prior to any procedure impacting the integrity of the urothelium and pre-treating in a culture-directed manner with a goal to improve antibiotic stewardship.1,2 However, it remains unclear when asymptomatic bacteriuria is clinically significant. The authors of this retrospective, single-institution study aimed to answer this question in the setting of urethroplasty by identifying which bacteria pose a higher risk for complications, i.e. (Source: Urology)

Editorial Comment   on “Prostate Cancer Detection Rate of Transperineal Prostate Biopsy: Cognitive vs Software Fusion, A Multicenter Analysis”
In this study, the authors compare cognitive and software fusion approaches to transperineal prostate biopsy. The authors retrospectively assess a multi-institutional cohort of 201 cognitive and 289 software fusion biopsies, with detection of clinically significant prostate cancer (csPCa) (i.e., Grade Group  ≥ 2) as the primary outcome. Analysis did not demonstrate statistically different odds of detecting csPCa by biopsy technique. (Source: Urology)

Prostate Cancer Detection Rate of Transperineal Prostate Biopsy: Cognitive vs Software Fusion, A Multicenter Analysis
To compare clinically significant prostate cancer detection with TP-TBx utilizing software vs cognitive fusion. It is established that MRI prior to prostate biopsy improves detection of clinically significant cancer (csPCa, Grade Group ≥2). MRI/US fusion targeted biopsy via a transperineal approach (TP-TBx) is increasing in utilization due to the clean percutaneous approach that greatly reduces postbiopsy infection. However, the comparative effectiveness of formal software fusion over cognitive fusion remains under studied. (Source: Urology)

Response to Comment on “Gender-Based Disparity Exists in the Surgical Experience of Female and Male Urology Residents”
We appreciate Dr. Westney ’s remarks on our work. We agree with the call for (1) additional research examining gender bias within urology training, (2) internal program auditing to identify inequality in training experiences, and (3) the development of methods to mitigate existing disparity in training. Our results identif ied a cumulative median difference of 99 cases between female and male residents across 4 years of training. We acknowledge program selection is a limitation to our methods, as the 13 included programs voluntarily submitted their case log data. (Source: Urology)
