Editorial Comment: “Access to Care and Healthcare Utilization Among Patients with Nephrolithiasis”
The article presents a compelling hypothesis on the determinants impacting access to healthcare among individuals diagnosed with nephrolithiasis. The primary outcomes were patients' self-reported healthcare access and utilization. The authors' study is timely in light of two recently published studies about access to health care in urology; these studies have underscored the impact of delays in stone treatment following stent placement, which is often influenced by variations in healthcare access. Editorial Comment on “Urinary and Fecal Incontinence During Sexual Activity Is Common and Bothersome Among Adults With Spina Bifida”
In a large online survey, Roth et al found that both most adults with spina bifida experience urinary incontinence and many experience fecal incontinence during sexual activity (70% and 45%, respectively).1 There are several important implications of their results. First, this is the first large study to demonstrate that urinary incontinence during sexual activity occurs in men and not just women. Indeed, 62% of men in this study experienced urinary incontinence.1 Ironically, although studies have been conducted on sexual function in men with spina bifida, they have failed to capture this highly bothersome aspect of their ...

Editorial comment on: “Clinical efficacy evaluation of a novel negative pressure ureteroscopic lithotripsy for ureteral stones”
There have been numerous advances in the treatment of nephrolithiasis including miniaturized instrumentation and new laser technology; however, the purpose of the procedure remains the same – to render the patient stone free. Recent publications reemphasizes the need to render patients stone free with residual fragment size contributing to stone-related events for both percutaneous nephrolithotomy1 and ureteroscopy.2 Both the vacuum assisted ureteral and percutaneous access sheaths a re now commonly used with advertised benefits of a reduction in stone retropulsion, shorter operating room times and potentially reducing p...

Pelvic Neuroblastoma of the Pediatric Prostate
We present a 4-year-old patient with pelvic neuroblastoma arising from the prostate and describe the medical and surgical management of this challenging case. While pelvic neuroblastoma may have an improved prognosis, this case demonstrates the challenging surgical decisions that accompany these patients to maintain quality of life while balancing oncologic efficacy of treatment. Pigmented Epithelioid Melanocytoma of the Penis: A Case Report
Pigmented epithelioid melanocytoma (PEM) is rare cutaneous melanocytic proliferation, characterized by heavily pigmented melanocytes, with a potential for regional node metastasis, but with an overall favorable clinical course. Here, we describe an uncommon case of PEM involving the penis in an adolescent patient. (Source: Urology)

Clinical efficacy evaluation of a novel negative pressure ureteroscopic lithotripsy for ureteral stones
We thank the Editorial Comment for recognizing our work on the novel negative pressure ureteroscopic lithotripsy (NP-URL). This method, integrating a tee joint and a 5F ureteric catheter, is designed to offer a more efficient and safer alternative to standard practices, focusing on reduced retropulsion and lower intrarenal pressures. It represents a significant step forward in making ureteroscopic lithotripsy both cost-effective and accessible, especially in resource-constrained environments. The editorial's recognition underscores the potential of NP-URL to improve stone-free rates and reduce complications without the nee...

Editorial Commentary on “Racial Disparities in Diagnosis and Treatment of Depression Associated with Androgen Deprivation Therapy for Prostate Cancer”
Racial disparities in prostate cancer treatment and their subsequent impact on outcomes including mortality have been well described. In addition to lower screening rates, inequities in guideline-concordant treatment, and lower representation in clinical trials,1 there are also disparities in health-related quality of life related to prostate cancer treatment in Black men,2,3 of which androgen-deprivation therapy (ADT)-associated depression is a major component. Editorial Comment on “Refining Bacteriuria as a Risk Factor for Complications After Urethroplasty: Identifying the Culprit”
In addition to standard peri-operative prophylaxis, both the American Urological Association and the Infectious Diseases Society of America recommend screening for asymptomatic bacteriuria prior to any procedure impacting the integrity of the urothelium and pre-treating in a culture-directed manner with a goal to improve antibiotic stewardship.1,2 However, it remains unclear when asymptomatic bacteriuria is clinically significant. The authors of this retrospective, single-institution study aimed to answer this question in the setting of urethroplasty by identifying which bacteria pose a higher risk for complications, that ...
