A Master Protocol Template for Pediatric ARDS Studies
CONCLUSIONS: The majority of pediatric ARDS trials identified were small and heterogeneous in study design and outcome reporting. Using a master protocol template for pediatric ARDS trials with CDEs would support combining and comparing pediatric ARDS trial findings and increase the knowledge base.PMID:38688543 | DOI:10.4187/respcare.11839 (Source: Respiratory Care)

Assessment of Ventilation Using Adult and Pediatric Manual Resuscitators in a Simulated Adult Patient
CONCLUSIONS: Both pediatric- and adult-sized BVM provided lower ventilation volumes than those recommended by professional guidelines for an adult. Ventilation with the pediatric BVM was significantly worse than with the adult bag when ventilating a simulated adult subject.PMID:38688544 | DOI:10.4187/respcare.11588 (Source: Respiratory Care)

Pulmonologist Education of the Teach-to-Goal Inhaler Technique for Those With Asthma and COPD
CONCLUSIONS: Pulmonologist education of inhaler technique for patients with asthma and patients with COPD was associated with decreased relative risk for inhaler/dosage increase and increased odds for symptom control. We recommend pulmonologists provide education of inhaler technique to patients with asthma and patients with COPD and not rely on non-pulmonologist education alone. Prospective research is needed to confirm the importance of proper inhaler techniques.PMID:38688545 | DOI:10.4187/respcare.11478 (Source: Respiratory Care)

Reliability and Validity of Maximal Respiratory Pressures
CONCLUSIONS: Only concurrent validity of maximal respiratory pressure measured with the manometers evaluated in this review presented a high level of evidence. The quality of clinical studies by using maximal respiratory pressure would be improved if more high-quality studies on measurement properties, by following well established guidelines and the COSMIN initiative, were available.PMID:38688546 | DOI:10.4187/respcare.10641 (Source: Respiratory Care)

Psychosocial Support in Pulmonary Rehabilitation
Respir Care. 2024 Apr 30:respcare.11850. doi: 10.4187/respcare.11850. Online ahead of print.ABSTRACTPulmonary rehabilitation (PR) improves exercise capacity and quality of life (QOL) while reducing dyspnea in patients with COPD. However, little is known about the efficacy of PR, cognitive behavioral therapy (CBT), or antidepressant drug therapy on psychosocial factors in patients with COPD. Knowledge gaps include which therapy is most efficacious, what barriers exist for each treatment, and the optimal duration of each intervention. Potential barriers to antidepressant therapy include patient fears of potential adverse eff...

Pulmonary Rehabilitation Reimbursement Challenges
Respir Care. 2024 Apr 30:respcare.11699. doi: 10.4187/respcare.11699. Online ahead of print.ABSTRACTPulmonary rehabilitation (PR) is a highly effective intervention for persons with chronic respiratory diseases, resulting in improvement in exercise capacity, dyspnea, health-related quality of life, mood, reduced hospitalization, and improved survival and cost savings post-COPD hospitalization. Despite demonstrated effectiveness, PR is underutilized in part due to lack of awareness, limited access, and inadequate PR reimbursement. Poor payment is a long-standing barrier to PR's financial stability and access. Addressing PR ...

Assessing the causal role of physical activity and leisure sedentary behaviours with chronic obstructive pulmonary disease: a Mendelian randomisation study
CONCLUSIONS: Our study suggests a potential association with LSB, particularly television watching, is associated with higher odds of COPD and lower indices of lung function as measured continuously, including FEV1 and FVC. Conversely, an increase in MVPA is associated with higher indices of lung function, particularly reflected in increased FVC levels.PMID:38688688 | DOI:10.1136/bmjresp-2023-001879 (Source: Respiratory Care)

The diagnostic pathway for patients with interstitial lung disease: a mixed-methods study of patients and physicians
CONCLUSIONS: This mixed-methods study identified patient and system-related factors that contribute to diagnostic delays for patients with ILD, with most delays occurring prior to respirology referral. ILD awareness and education, earlier presentation to primary care, expedited access to chest imaging and earlier referral to respirology may expedite diagnosis.PMID:38688689 | DOI:10.1136/bmjresp-2024-002333 (Source: Respiratory Care)

Reducing short-acting beta-agonist use in asthma: Impact of national incentives on prescribing practices in England and the findings from SENTINEL Plus early adopter sites
NPJ Prim Care Respir Med. 2024 Apr 29;34(1):6. doi: 10.1038/s41533-024-00363-0.ABSTRACTShort-acting beta-agonist (SABA) over-use in asthma is harmful for patients and the environment. The Investment and Impact Fund (IIF) 2022/2023 financially rewarded English primary care networks that achieved specific targets, including reducing SABA over-use (RESP-02) and lowering the mean carbon footprint per salbutamol inhaler prescribed (ES-02). SENTINEL Plus is a co-designed quality improvement package that aims to improve asthma outcomes and reduce asthma's environmental impact by addressing SABA over-use. We investigated the impac...

The journey of lung cancer patients from symptoms to diagnosis in Greece. A mixed methods approach
This study aimed to (a) track the patients' journey from the onset of symptoms until diagnosis and, (b) explore the patients' perspective of the journey until diagnosis, on the largest island of Greece. A convergent mixed-methods study was conducted with 94 patients with lung cancer. Patients completed a self-report questionnaire and were interviewed about their symptoms and journey through the healthcare system before their diagnosis. Our findings revealed several problems and delays in the diagnostic process. Both quantitative and qualitative data showed that patients did not recognize their symptoms and sought medical a...

Acute respiratory failure due to pulmonary exacerbation in children with cystic fibrosis admitted in a pediatric intensive care unit: outcomes and factors associated with mortality
CONCLUSION: Children with CF admitted to PICU for acute respiratory failure secondary to pulmonary exacerbations are at high risk of death, both in the short and medium terms. Lung transplant is their main chance of survival and should be considered early.PMID:38685088 | PMC:PMC11059703 | DOI:10.1186/s12931-024-02778-2 (Source: Respiratory Care)
