Understanding factors affecting implementation success and sustainability of a comprehensive prevention program for cardiovascular disease in primary health care: a qualitative process evaluation study combining RE-AIM and CFIR
Prim Health Care Res Dev. 2023 Mar 8;24:e17. doi: 10.1017/S1463423623000063.ABSTRACTAIM: Our aim was to evaluate the implementation process of a comprehensive cardiovascular disease prevention program in general practice, to enhance understanding of influencing factors to implementation success and sustainability, and to learn how to overcome barriers.BACKGROUND: Cardiovascular disease and its risk factors are the world's leading cause of mortality, yet can be prevented by addressing unhealthy lifestyle behavior. Nevertheless, the transition toward a prevention-oriented primary health care remains limited. A better underst...

Agreement between rapid antigen detection test and culture for group A streptococcus in patients recently treated for pharyngotonsillitis - a prospective observational study in primary care
CONCLUSION: RADT and culture for GAS agreed to a high extent also after recent penicillin V treatment. RADT for GAS means a low risk for missing the presence of GAS.KEY POINTSTesting for group A streptococci (GAS) before antibiotic treatment can reduce antibiotic prescription for pharyngotonsillitis. It has been proposed that rapid antigen detection tests (RADT) for group A streptococci after recent penicillin V treatment may be falsely positive due to possible persisting antigens from non-viable bacteria. The decline of the presence of GAS was similar between RADT and conventional throat culture in patients who had recentl...

Development of the Cardiovascular Assessment Screening Program (CASP) using the qualitative findings of a mixed methods study and applying the TDF to address the barriers of and facilitators to comprehensive screening for cardiovascular disease
CONCLUSION: CASP is a theory-informed intervention developed through the integration of the findings from the focus groups and interviews with selected TDF domains, behaviour change techniques, and modes of delivery available in the local context that may be a useful approach for knowledge translation of evidence into practice.PMID:36882713 | DOI:10.1186/s12875-023-02022-8 (Source: Primary Care)

The predictive value of weight evolution in screening for malnutrition in community-dwelling older persons (70+) in Antwerp
CONCLUSION: In this study, weight evolution has a low sensitivity in screening for (risk of) malnutrition in people over 70 living at home compared to the MNA-SF. However, in order to detect people with malnutrition, this study demonstrated a sensitivity of 71.4% and a specificity of 92.3% for a weight loss of ≥ 5% at six months.PMID:36879209 | PMC:PMC9987120 | DOI:10.1186/s12875-023-02020-w (Source: Primary Care)

The indirect impact of the COVID-19 pandemic on people with type 2 diabetes mellitus and without COVID-19 infection: Systematic review and meta-analysis
CONCLUSIONS: This review did not find significant changes in HbA1c and BMI among people with T2DM after data pooling, but a possible worsening in lipids parameters during the COVID-19 pandemic. There were limited data on long-term outcomes and healthcare utilization, which warrants further research.SYSTEMATIC REVIEW REGISTRATION: PROSPERO CRD42022360433.PMID:36872178 | PMC:PMC9977626 | DOI:10.1016/j.pcd.2023.02.006 (Source: Primary Care)

Community health workers (Behvarz) in primary health care: a qualitative inductive content analysis of challenges
CONCLUSION: Occupational challenges affect the performance of Behvarzs in responding to society's needs because they not only play a major role in the health system, but also contribute to addressing the communication gap between local communities and high-level institutions, leading to the alignment of policy implementation. Therefore, strategies that emphasise the role of Behvarzs should be followed to promote community engagement.PMID:36872455 | DOI:10.1071/PY22052 (Source: Primary Care)

Experiences of colorectal cancer survivors in returning to primary coordinated healthcare following treatment
CONCLUSION: There is a need for improved discharge preparation and information for GPs, and earlier recognition of concerns following CRC treatment to ensure timely management and access to services in the community, supported by system-level initiatives and appropriate interventions.PMID:36872459 | DOI:10.1071/PY22201 (Source: Primary Care)
