Challenges of pediatric pharmacotherapy: A narrative review of pharmacokinetics, pharmacodynamics, and pharmacogenetics
AbstractPurposePersonalized pharmacotherapy, including for the pediatric population, provides optimal treatment and has emerged as a major trend owing to advanced drug therapeutics and diversified drug selection. However, it is essential to understand the growth and developmental characteristics of this population to provide appropriate drug therapy. In recent years, clinical pharmacogenetics has accumulated knowledge in pediatric pharmacotherapy, and guidelines from professional organizations, such as the Clinical Pharmacogenetics Implementation Consortium, can be consulted to determine the efficacy of specific drugs and ...

Selective androgen receptor modulator use and related adverse events including drug-induced liver injury: Analysis of suspected cases
ConclusionPromoting SARMs as an anabolic agent in combination with other performance-enhancing drugs poses a risk to users not only due to doping controls but also to health safety. The lack of quality control of consumed supplements makes it very difficult to assess the direct impact of SARMs on the liver and their potential hepatotoxic effects. Therefore, more detailed analyses are needed to determine the safety of using SARMs. (Source: European Journal of Clinical Pharmacology)

Pharmacokinetics of oral spironolactone in infants up to 2  years of age
ConclusionThe pharmacokinetics of spironolactone and its metabolites was highly variable between patients below 2  years of age. Body weight explained a significant part of this variability; this highlights the need to take it into account for dosing prescription in this population. (Clinical trial Registration Number 2013–001189-40). (Source: European Journal of Clinical Pharmacology)

Anticoagulants in adult extracorporeal membrane oxygenation: alternatives to standardized anticoagulation with unfractionated heparin
ConclusionDiverse anticoagulant options are available and under investigation for ECMO. These alternatives may enhance patient safety and outcomes during ECMO support. Further research and clinical studies are warranted to determine their effectiveness and safety profiles. (Source: European Journal of Clinical Pharmacology)

Differential inhibition of platelet function by cilostazol in combination with clopidogrel
ConclusionsThe inhibitory effects of adjunctive cilostazol added to clopidogrel on platelet function differed by type of platelet function test. VerifyNow % inhibition and PRI were more inhibited than the other platelet function tests.Trial registrationCSPS.com substudy in TWMU (UMIN000026672), registered on April 1, 2017. This study was performed as a substudy of CSPS.com (UMIN000012180, registered on October 31, 2013) and was retrospectively registered. (Source: European Journal of Clinical Pharmacology)

The impact of a summative national prescribing assessment and curriculum type on the development of the prescribing competence of junior doctors
ConclusionOur findings suggest that the inclusion of the knowledge focused DNPA in the medical curriculum improves the prescribing knowledge, but not the skills, of junior doctors at the moment of graduation. However, after 6 and 12  months, both the knowledge and skills were higher in the junior doctors whose medical curriculum included the DNPA. A curriculum that provides both theoretical and practical education seems to improve both prescribing knowledge and skills relative to a solely theoretical curriculum. Clinical comprehensive evaluation of direct oral anticoagulants for patients with atrial fibrillation in China
ConclusionsThis study provided a scoring framework developed for comprehensive evaluation of DOACs in China. The ranking of DOACs could help to support the decision-making in clinical practice. The framework could provide a reference for comprehensive evaluation of other drugs. (Source: European Journal of Clinical Pharmacology)

Serotonin syndrome: A pharmacovigilance comparative study of drugs affecting serotonin levels
ConclusionPrescribers need to be vigilant about drugs that can raise serotonin concentration or influence serotonergic neurotransmission, also when using drugs with less well-known risk for serotonin syndrome, like linezolid and triptans. (Source: European Journal of Clinical Pharmacology)
