Urinary Ascites and Transient Intestinal Obstruction in a Preterm Infant: An Interesting Case of Posterior Urethral Valve
We report a preterm neonate with PUV who presented as nonimmune fetal hydrops with intestinal obstruction in the antenatal period. The mother of our patient is a 33-year-old woman who started her prenatal care at our hospital at 30 weeks' gestation. Her sonogram done at 32 weeks in our hospital revealed fetal hydrops. It showed polyhydramnios, mild pyelectasis of right kidney, normal left kidney, and fetal ascites. Amniocentesis revealed bile stained amniotic fluid. Ultrasound during the procedure showed dilated fetal bowel loops with increased echoes. Following delivery at 32 weeks postnatal exam showed ascites with absen...

Cerclage Location and Gestational Age at Delivery
Conclusions For Hx and PE indicated cerclages, the location of the stitch may influence the timing of delivery. Specifically, the higher the cerclage, the more advanced the gestational age at delivery. Operative Vaginal Delivery Is a Safe Option in Women Undergoing a Trial of Labor after Cesarean
Conclusion The success rate of OVD is high in TOLAC with similar outcomes to RCD. Maternal composite outcomes were highest with forceps-assisted vaginal deliveries. However, considering overall morbidity, OVD in the second stage of labor in TOLAC is a reasonable, safe option in selected cases. Targeted Neonatal Echocardiography-Guided Therapy in Vein of Galen Aneurysmal Malformation: A Report of Two Cases with a Review of Physiology and Approach to Management
AJP Rep 2019; 09: e172-e176 DOI: 10.1055/s-0039-1688765Vein of Galen malformation results in predictable changes in physiology which exist on a continuum. Severe pulmonary hypertension may present as hypoxemia; however, excessive reduction in pulmonary vascular resistance may precipitate progressive pulmonary overcirculation and impaired systemic blood flow. Right ventricular performance and the patency and direction of the ductus arteriosus may play a crucial role in postductal organ perfusion. Physiological stabilization may be complex and variable over time. The utilization of targeted neonatal echocardiography to guide...

Babesiosis in Pregnancy: An Imitator of HELLP Syndrome
AJP Rep 2019; 09: e147-e152 DOI: 10.1055/s-0039-1687873HELLP syndrome (hemolysis, elevated liver enzymes, and low platelets) is a serious pregnancy complication that can cause significant maternal and neonatal morbidity and mortality. There are several conditions that may occur in pregnancy that may imitate the laboratory findings and clinical presentation of HELLP syndrome. Babesiosis is a parasitic imitator of HELLP syndrome that can be spread by the tick, transfusions, or congenitally. Recognition and treatment of this condition is important to optimize maternal and fetal outcomes. [...] Thieme Medical Publishers 333 Se...

Effect of a High-Fat Diet and Metformin on Placental mTOR Signaling in Mice
Conclusion Increased placental mTOR signaling and metformin inhibition of placental mTOR signaling only occurred in the presence of an HFD exposure. These findings suggest that metformin may modulate placental mTOR signaling in the presence of metabolic exposures during pregnancy. Pentalogy of Cantrell in Full Term Neonate
AJP Rep 2019; 09: e144-e146 DOI: 10.1055/s-0039-1683936Pentalogy of Cantrell (PC) is an uncommon congenital disorder characterized by severe defects in the chest and abdomen, including abdominal visceral prolapsed via umbilical cord (omphalocele), defect in the lower part of the sternum, defect in the front of the diaphragm, defects in the anterior part of the pericardium, and the ectopiacordis. Here, we report a 2-hour-old girl, weighing 3,500 g, who was referred to Shahid Madani Hospital in Khorramabad due to the large omphalocele on her chest with pulsating mass above it. The baby was the first child of a 24-year-old ...
