Typical overweight female with classic symptoms. POCUS with large ONSD. How many of you guys are doing the LP in the ER? Any evidence that this needs to be done emergently? Do you admit for MRI/MRV? Do you start them on acetazolamide before confirming dx? (Source: Student Doctor Network)

Case discussion
Had a weird case about a month ago, curious to see what others would do, and differential diagnosis, investigative workup etc. Identifying details have been changed to protect confidentiality. I will start the post with all the details when I took over her care. I can post the autopsy results at the end. This was a 77 year old woman, admitted with acute abdominal pain. PMHx of hypertension and gout, appy/chole. Living independently, married, non smoker, reasonably healthy lady. CT scan of... Case discussion (Source: Student Doctor Network)

How did your aortic dissection cases present?
My mind is racing because I’ve seen two aortic dissections in the last week and I feel like I could have easily missed them had I been in a rush and not prodded during the history. Makes me cringe thinking about it. First one was a 69 year old with hypertension. Sudden onset of chest pain when watching TV. With no other symptoms at all. The thing that tipped me off is he didn’t want to move. Moving hurt his chest. Breathing hurt his chest. Me pushing on his chest made him wince. Just didn’t... Isolated Systolic Hypertension
Hello, there! Our Therapeutics prof told us that "Isolated Systolic Hypertension" or ISH is somehow "more dangerous" than the Isolated Diastolic Hypertension and it is a more reliable predictor of the development of future cardio-vascular diseases. Why's that? Is it because during systole the heart is contracting and therefore it would "get tired" too much and too quickly if the blood pressure's high? Thanks so much in advance! Metabolic medicine?
I read that chemical pathologists can specialize in metabolic medicine. I just stumbled accross this yesterday and found it quite intriguing My questions are: What do you guys treat and do, typical work and patient contact? All I hear its a relatively newer subspecialty and that they can treat any metabolic manifestations and are involved in parenteral nutrition. There's a fair amount of lab and patient contact which sounds cool. Involved in treating diabetes, hypertension, obesity... Metabolic medicine? (Source: Student Doctor Network)
