Anorectal varices and hemorrhoids difference
Hey, In portal hypertension, blood is shunted from the superior hemorrhoidal vein to the middle and inferior rectal veins. However, Pathoma claims that the portosystemic shunts include hemorrhoids. Is this correct? I mean, definition of hemorrhoids is dilated veins either above or below the dentate line. So it seems that anorectal varices just fall into this broad general category that is hemorrhoids. Can I reason this way or is this wrong? (Source: Student Doctor Network)

Hi - 1) Why does destruction of "lung parenchyma" in COPD lead to pulmonary hypertension? The way I am envisioning it, is that there is simply less tissue so the blood vessels have less resistance and should therefore NOT lead to hypertension. Perhaps you can paint a better picture for me. 2) Shunting increases the A-a gradient as mentioned in First Aid. The seminal study: No enhanced risk of cancers with hypertension, reduced risk of cancers with higher cholesterol. Increased risk of cancer with Congestive cardiac failure needs to be added.

Magnesium C section
hi folks. had a patient with gestational hypertension (no evidence of proteinuria) who had Mg running as per OB service. pt was brought to the OR for fetal intolerance/decels. spinal was placed and by this time i had stopped the Mg infusion since i didn't think pt had preeclampsia and there was no need for seizure prophylaxis. anyways the OB was not happy i had stopped the infusion and told me that the patient was still high risk for seizures. i know this is true for pre-eclamptics but... Pulmonary Hypertension Devices
So for pHTN we have: inhaled NO inhaled milrinone inhaled Ilioprost (carbacyclin deriv of Pg I2) inhlaed flolan (also PgI2 ) Does anyone have experience with the devices used to nebulise these in the OR. Adjuvant RT for SCC of the vulva
Dear friends, I have a 68 yo lady (diabetic and has hypertension) that was diagnosed of right vulvar SCC grade 1, pT1bpN0. 4cm lesion, 1cm Depth of invasion, no LVSI, all margins are negative > 8 mm except for urethral margin which is at 7 mm. Which is first: HF or HTN?
How to differentiate between: 1. Hypertension causing hypertrophy and eventual heart failure and congestion and 2. congestive heart failure for another reason causing decreased renal blood flow and increased RAAS leading to HTN? I came by a Q in UW (qid: 1532) where they show a patient presenting with CHF and HTN and ask about angiotensin II levels in his pulmonary veins.
