Family Physician looking for Uegrnt Care / Fast Track / other work in the ER
Hey folks I would like to work in the Fast Track in the ER, love working with other colleagues (one reason I want to work in the ER rather than urgent care on my own), also want to see higher acuity patients and have the ER Doc's in case I can want to consult them (level 3 patients like headaches, abdo pain, chest pain etc ) It is hard to find these jobs in databases (practice match etc), I have looked under jobs for Family Docs and ER physicians and I only got 1 hit that was out of date... Decadron burned my patients crotch?
Anyone ever hear of this? I was treating migraine. 10mg IV decadron, given alongside benadryl and reglan as well as gram of mag. After receiving all of those meds (and of the course the headache didn't improve), upon receiving the decadron the patient apparently sat bolt upright and complained of severe burning in the crotch region. What gives? is there an explanation for this? (Source: Student Doctor Network)

Anthem breaking with Express Scripts; partnering with CVS to form own PBM
Anthem Breaks With Express Scripts, Will Start Own Drug Plan It hasn't been a good week for Express Scripts... Are there any independent pharmacists left on this forum? If so: what do you think about this- is this a game changer, or just window dressing? (I'm told Optum [UnitedHealth's PBM] is a bigger headache than Express Scripts ever was) (Source: Student Doctor Network)

Frequent Flyers with Migraines
At my shop, headache seems to be the most common complaint among habitual visitors. Headaches seem to rival and even outnumber our back painers. We have several customers who present 3+ times weekly, twice in the same 12 hr shift, 200+ visits per year. Naturally, toradol, other NSAIDS and every triptan under the sun are listed as an allergy. Although it seems at times instinctive to order them their shot of dilaudid and phenergan, this is sometimes more of a knee jerk reaction in order... Not sure where to put this
Hello fellow medical professionals.. Last year I wanted to have start graduate school again. I want to go to Pharm school or nursing or something in the medical field. Right now I feel like those dreams are gone. The reason is I have been having these tension headaches for the past one month (or more) and they are not going away. Simple noises such as a car noise or sitting in a car (I think the cause is the pressure of the moving car) my head hurts or ache (dull pain) and I don't know... What would you do?
Patient is a female 30 years old. Has seen other psychiatrists in the past all with the same diagnosis: schizoaffective depressive type. Takes Zyprexa and Paxil and Topamax for migraines. She is on disability and going to school. She says she hopes to work for a bank. You continue treatment she appears to be doing well. She hits a bump in the road and almost dies. Turns out she was drinking her way through college. (Medical report shows her lipase rose to 1200). She goes to rehab and comes... What would you do? (Source: Student Doctor Network)

Medicine Sucks
Had to tell a family "Yeah, the reason your 8 yo daughter has been having those headaches is because she's got a big old brain tumor." Medicine sucks. (Source: Student Doctor Network)

Cloudy Intrathecal medication
Have you guys seen this before? 2nd refill on same patient with intrathecal hydromorphone looking very cloudy when it is withdrawn from the pump. Have sent it off twice now for infectious work up which has come back negative. She did have a headache with mild fever about a month ago; work up in ER was negative for meningitis etc... All of our pump meds come from the same pharmacy and are never expired when we put them in. Patient is very reliable and hasn't had any aberrant behaviors. I... Scope of Practice in Manual Therapy for a Physical Therapist
I am an undergraduate student currently looking into the PT profession and I am most interested in manual therapy/ortho. I've read the APTA manipulation/mobilization articles and they state that there is conflict between chiropractors and physical therapists in that area. I have been to a chiropractor before (I felt that he was evidence-based and got rid of my headaches) and I am really interested in the way that they can "adjust" the spine and neck to get people to feel better. I know... Scope of Practice in Manual Therapy for a Physical Therapist (Source: Student Doctor Network)

Another "Do I Have A Chance", very anxious right now...
I'm really anxious right now. Do you guys think my stats would secure me a chance at Rowan (my state school)? Or schools like NYIT, PCOM and Touro. Those are my top 4. I have sent out secondaries but very anxious right now and has started losing sleep over it. These stress give me headache and hives and make me sick everyday. So I just want a little advice and what to prepare next. I went to state university for undergrad, s/cGPA on AACOMAS is 3.92, on my transcripts was 3.659. Don't know...
