CSE and spinal headache
New attending and recently I've been switching to doing CSE for my labor epidural. Unfortunately lately I've had a significant increase in spinal headaches. When i pull the spinal needle, no CSF from the tuohy, cathether passess easily and no response to the test dose. No sign of inadvertent dural puncture. I typically use a 25g spinal needle but now im thinking of switching to a27g. My understanding is that CSE should actually decrease the risk of PDPH but maybe I've been unlucky. Any... Occipital Nerve PNS
Hey All, Has anyone here done a peripheral nerve stimulator for Occipital Neuralgia? I have a patient who has done every medical and almost every interventional option for his occipital neuralgia as well as seen tons of academic headache specialists. Nothing has worked. Endless TV shows about heroic, idealized portrayals of physicians. Why none about psychologists? Grinds my gears a bit.....
Can anyone explain this to me? We deal with all sorts of crazy headaches, have insane training to deal with, nutsiness amongst colleagues, but plenty (IMHO) to romanticize and lionize about our field as well. There's all sorts of ridiculous TV shows about physicians. American TV seems to have as much of an obsession about doctor-related TV shows as they do about zombie-related TV shows. How to avoid a headache?
Working on secondaries currently. Also have a tech job where I’m in front of a computer for ~8 hours a day. Overall, I’m spending upwards of 14 hours/day in front of a screen, whether for med school apps or tech work. I end up with a dull, throbbing headache everyday. Any tips to avoid this? Status Migrainosus
Anyone ever use propofol in these patients? There are a spat of studies showing bolusing small doses of propofol can help. I've been angling to do this...and finally did the other day. neurology infusions
What things in neurology can you routinely do infusions for? just headaches and ms? Also, are infusions becoming more routine in private practice, and wouldn't more infusions equal more $$? Anyone who loved obgyn but didn't pursue it? What did you do?
I am on my very nice family med rotation, which I was hoping to love partially due to my low Step 1 but also due to the opportunity to do obgyn. I unfortunately find that I feel overwhelmed with the case load of patients in their 50/60s with comorbid conditions and the fact that the PCP is often the first to field an undifferentiated complaint like back pain or headaches.
