Endoscopic incision of the duodenal descending incomplete obstruction caused by duodenal web (with video)
A 61-year-old woman was referred to our hospital because she had experienced postprandial epigastric satiety for 10 years. The GI barium study showed a pocket-like structure in the descending duodenum (A, arrow), and contrast-enhanced CT of the abdomen showed dilatation of the descending part of the duodenum with obstruction. A membranous stenosis with a small pinhole opening was observed in the descending part of the duodenum, and the papilla of Vater was observed in the medial wall by endoscopy (B, arrow). A pathological complete response after immunotherapy with pembrolizumab for distal duodenal adenocarcinoma caused by Lynch syndrome: a case report
We present the case of a 55-year-old woman who was diagnosed with duodenal cancer. Biopsy findings revealed MSI-H, and pembrolizumab therapy was initiated because the tumor was in contact with the left renal vein and had metastasized to the mesenteric lymph nodes of the small intestine. Subsequently, after completing two courses of pembrolizumab therapy, the patient developed duodenal stenosis and underwent surgery. Various imaging modalities, including computerized tomography, endoscopic ultrasound, and magnetic resonance imaging, have a valuable role in characterizing groove pancreatitis.
