Clinical Pathology Conference: Case 4: An exophytic gingival mass of the right anterior mandible
An 83-year-old male presented to a community oral maxillofacial surgeon with a tender, pink, pedunculated exophytic tumor measuring 2  × 1.5 cm involving the right anterior mandible adjacent to tooth #27 (Figure 1A). His medical history was significant for hypertension, congestive heart failure, atrial fibrillation, and prostate cancer treated with radiation and androgen deprivation chemotherapy. The patient had been taking a miodarone, metoprolol, digoxin, rivaroxaban, furosemide, bicalutamide, and leuprorelin injections. Prevalence and associated risk factors of resistant hypertension among Chinese hypertensive patients in primary care setting
CONCLUSION: The prevalence of RHT among Chinese hypertensive patients in primary care setting of Hong Kong is 7.43%. RHT is more common in male patients, patients with longer duration of hypertension, concomitant T2DM and CHF. Clinicians should be vigilant when managing these groups of patients and provide aggressive treatment and close monitoring.PMID:38641566 | PMC:PMC11027357 | DOI:10.1186/s12875-024-02366-9 (Source: Primary Care)

Newer Pharmacologic Treatments in Adults With Type 2 Diabetes: A Clinical Guideline From the American College of Physicians
Ann Intern Med. 2024 Apr 19. doi: 10.7326/M23-2788. Online ahead of print.ABSTRACTDESCRIPTION: The American College of Physicians (ACP) developed this clinical guideline to update recommendations on newer pharmacologic treatments of type 2 diabetes. This clinical guideline is based on the best available evidence for effectiveness, comparative benefits and harms, consideration of patients' values and preferences, and costs.METHODS: This clinical guideline is based on a systematic review of the effectiveness and harms of newer pharmacologic treatments of type 2 diabetes, including glucagon-like peptide-1 (GLP-1) agonists, a ...

An assessment of the association between patient characteristics and timely lung cancer treatment
ConclusionsWithin a Medicare Advantage population, patient demographics were found to be significantly associated with the decision to pursue surgery, but factors other than stage were not significantly associated with time to non-systemic treatment. (Source: Cancer Causes and Control)

Association of device detected atrial and ventricular tachyarrhythmia with adverse events in patients with an implantable cardioverter ‐defibrillator
ConclusionOur findings from a large cohort of ICD recipients enrolled in landmark clinical trials show that device detected AF and VT/VF can be used to identify patients with increased risk for CHF exacerbation and mortality. These findings suggest a need for early intervention in CHF patients who develop device-detected atrial and ventricular tachyarrhythmias. (Source: Journal of Cardiovascular Electrophysiology)

NSQIP 5-factor modified frailty index and complications after ileal anal pouch anastomosis for ulcerative colitis
CONCLUSION: IPAA for CUC is associated with high postoperative morbidity, however, the mFI-5 alone has limited utility in determining which patients are at a higher risk of complications due to frailty. These observations suggest there is a need for more relevant instruments to measure frailty in this patient cohort.PMID:38601734 | PMC:PMC11004715 | DOI:10.1016/j.sopen.2024.03.011 (Source: Anal Sci)

Severe Community-Acquired Pneumonia: Noninvasive Mechanical Ventilation, Intubation, and HFNT
The objective of all the supporting measures for ARF is to gain time for the antimicrobial treatment to cure the pneumonia. There is uncertainty regarding which patients with severe CAP are most likely to benefit from each noninvasive support strategy. HFNT may be the first-line approach in the majority of patients. While NIV may be relatively contraindicated in patients with excessive secretions, facial hair/structure resulting in air leaks or poor compliance, NIV may be preferable in those with increased work of breathing, respiratory muscle fatigue, and congestive heart failure, in which the positive pressure of NIV may...

Neighborhood socioeconomic disadvantages associated with increased rates of revisions, readmissions, and complications after total joint arthroplasty
ConclusionOur findings of higher complications of the ADI  >  47 cohort at 90 days, reaffirm the complex relationship between ADI, patient demographics, and additional socioeconomic parameters that may influence postoperative outcomes and complications after TJA. This study utilizing ADI demonstrates potential areas of intervention and further investigatio n for assessing arthroplasty outcomes. Factors Associated With Episode of Care Adverse Events After Humerus Nonunion Repair
CONCLUSION: Using a large database over a recent 10-year period, we identified demographic and comorbid factors independently associated with episode of care adverse events after humeral nonunion repair. Patients 50 years or older had approximately three times the incidence of complications, readmissions, and reoperations in the first month after humeral nonunion repair compared with patients younger than 50 years. Our findings are relevant for preoperative risk stratification and counseling. [Orthopedics. 202x;4x(x):xx-xx.].PMID:38568000 | DOI:10.3928/01477447-20240325-03 (Source: Cancer Control)

Prescribing Benzodiazepines and Opioids and Clinical Characteristics Associated With 30-Day Hospital Return in Patients Aged ≥75 Years: Secondary Data Analysis
CONCLUSION: Findings highlight the importance of hospital nurses' role in identifying high-risk patients, educating patients and caregivers, monitoring them closely, communicating with primary care physicians and specialists, and conducting intensive follow up via telephone to avoid 30-day rehospitalization.
