CMS Prescription Trends for Quetiapine are Increasing: Can This Be Secondary to Off-Label Use?
Quetiapine is an atypical antipsychotic that is FDA approved for the treatment of schizophrenia, acute mania in bipolar 1 disorder, bipolar depression, and as an adjunct in major depressive disorder (MDD). Likely due to quetiapine's broad action amongst neurotransmitter receptors, and favorable side effect profile compared to other antipsychotic medications, the use of quetiapine is widespread for a variety of off-label diagnoses. Here we present information on the trends of the prescription of quetiapine from the center for Medicare and Medicaid Services (CMS) database. (Source: The American Journal of Geriatric Psychiatry)

Clinical outcomes of transcarotid artery revascularization vs carotid endarterectomy from a large single-center experience
Transcarotid artery revascularization (TCAR) has been practiced as an alternative for both carotid endarterectomy (CEA) and transfemoral carotid artery stenting, specifically in high-risk patients. More recently, the Centers for Medicare and Medicaid Services expanded coverage for TCAR in standard surgical risk patients if done within the Society for Vascular Surgery Vascular Quality Initiative TCAR surveillance project. A few registry studies (primarily from the Society for Vascular Surgery Vascular Quality Initiative) compared the early and up to 1-year outcomes of TCAR vs CEA or transfemoral carotid artery stenting. (So...

Considerations and Recommendations for Hand Surgeons in Light of Centers for Medicare and Medicaid Services Price Transparency Rulings
In 2021, the Centers for Medicare and Medicaid Services (CMS) ruled that hospitals publish pricing data for at least 300 shoppable services, including 70 CMS-specified services. This ruling intended to improve patient choice, access to pricing information, and curb costs for healthcare consumers by encouraging free market behavior in an increasingly expensive healthcare economy. However, due to poor enforcement, data reporting, and compliance, the ruling neither improved existing pricing disparities nor patient choice. Health related social needs and social determinants of health: Navigating the convoluted path of health equity in surgery
Evidence demonstrates that social determinants of Health (SDOH) are associated with 70  % of health outcomes.1 Nationally, there is attention on unmet social needs to improve health equity, reduce costs, and impact patient outcomes.1 Coinciding with SDOH, Health-Related Social Needs (HRSNs) are the immediate social needs that affect the health outcomes for individuals, such as incom e instability, housing instability, and food insecurity.3 In 2023, the Joint Commission mandated health-related social needs (HRSN) screening and referral which impact SDOH, and in 2024, the Centers for Medicare and Medicaid Services (CMS) ad...

A Single Center Analysis of Palliative Care Clinician Contact Following Declination from Advanced Heart Failure Therapy
In 2013 the Center for Medicare and Medicaid Services (CMS) issued a National Coverage Determination that made palliative care involvement a requirement for those considered for Left Ventricular Assist Devices (LVADs) as a destination therapy.1 In response, many advanced heart failure therapy programs began incorporating palliative care consults into their LVAD and heart transplant (HT) workflow. Despite this, incorporation of palliative care clinicians into the earlier stages of heart failure care remains lacking. (Source: Journal of Cardiac Failure)

2024 Updates to American Medical Association ’s Current Procedural Terminology Codes for Oncology Panel Testing
Billing and reimbursement for medical services are codified by a code set, Current Procedural Terminology (CPT), that is maintained and administered by the American Medical Association (AMA). CPT codes are considered level 1 Healthcare Common Procedure Coding System which is overseen by the Centers for Medicare and Medicaid Services (CMS) (https://www.cms.gov/medicare/coding-billing/healthcare-common-procedure-system, last accessed 01/03/2024). CPT codes are assigned categories. Category I CPT codes are procedures that are consistent with contemporary medical practice, are widely performed, and demonstrate clinical validit...

Quarter Century Clinical PET/Computed Tomography: Transforming Medical Oncology Practice
It is nearly 100 years since basic scientific research led to advances in PET and over a quarter century since the US Centers for Medicare and Medicaid Services (CMS) approved reimbursement for PET for clinical investigation of pulmonary nodule. A prototype hybrid PET/computed tomographic (CT) scanner was developed, and clinical evaluation commenced in 1998. Hybrid PET/CT scanners were introduced to the clinical practice in 2001 and more recently the hybrid PET/MRI scanners. Evolution of PET in clinical use and in medical oncology has been an iterative process of incorporating tremendous advances in technology, radiochemis...

Asynchronous direct to patient teledermatology visits (e-visits) during COVID-19 pandemic: Lessons for future teledermatology practice
To the Editor: Teledermatology  became an essential mode of health care during the COVID-19 pandemic; this was facilitated by the Centers for Medicare and Medicaid Services 1135 waiver, providing parity compensation for telehealth services (synchronous visits) and allowing services to be rendered from patients’ homes via non-H ealth Insurance Portability and Accountability Act compliant platforms.1 We aimed to provide insights into diagnoses best suitable for direct to patient asynchronous teledermatology (e-visit), and sought to understand how demographics may contribute to the utilization of telehealth visits. Using Performance Status to Identify Risk of Acute Care Transfer in Inpatient Cancer Rehabilitation
Cancer survivors constitute a large and growing proportion of inpatient rehabilitation facility (IRF) admissions because of improved survival rates.1 According to the Centers for Medicare and Medicaid Services in the United States (US), 60% of IRF admissions must fall within specific rehabilitation impairment codes, which do not include cancer diagnoses.2,3 Therefore, cancer survivors are often admitted to IRF based on the relevant impairment from cancer rather than the primary cancer type. Cancer admissions to IRF vary along the entire oncology continuum, including functional impairment after initial diagnosis, complicati...

Message from Dr. Eric Howell: The SoHM Annual Report
This report has been a key resource for many years on hospital medicine group composition and finances, including salary and hospital financial support. Hospitalists now earn as much as $323,500 annually! But just as importantly, given the pivotal role played by hospitalists during the COVID-19 pandemic, SHM deployed a new, first-ever Workforce Experience Survey which captured patient load, perceived safety, and hospitalist well-being. With more than 560 survey responses, there was a mixed picture of how hospitalists are doing. About 68% of respondents reported feeling at least some burnout, but at the same time, 86% repo...

Seventy-five years of service: an overview of the College of American Pathologists ’ proficiency testing program in histocompatibility and identity testing
The Histocompatibility and Identity Testing Committee offers an overview of the College of American Pathologists’ (CAP) Proficiency Testing (PT) program, commemorating its significant 75th anniversary in 2024. The CAP PT program has undergone significant growth and evolution over the years, ultimately achieving Centers for Medicare and Medicaid Services approval. In 1979, CAP’s partnership with the American Association for Clinical Histocompatibility Testing marked a pivotal moment, leading to the creation of the first proficiency testing survey in 1980. This laid the foundation for various PT programs managed by the C...
