Risk factors for stent thrombosis
can be divided into those due to unfavourable anatomical characteristics or procedural characteristics and those due to unfavourable clinical characteristics Unfavourable anatomical / procedural characteristics at risk of stent thrombosis and life threatening complications: Long stented segments Multiple stents Overlapping stents Small vessels Bifurcation lesions Left main and last remaining vessel – more of a risk for life threatening situation if stent thrombosis occurs Unfavorable clinical characteristics at risk for stent thrombosis / life threatening complications: Recent acute coronary syndrome (ACS) History ...

Mitral annular calcification (MAC)
is more likely in those with chronic kidney disease because of the deranged calcium and phosphorus metabolism. Products of calcium and phosphorus metabolism cause soft tissue calcification. MAC is also a marker of atherosclerotic burden. Mitral annular calcification is associated with higher incidence of atrial arrhythmias, stroke and cardiovascular mortality. MAC is more common in females, elderly and in those with hypertension and diabetes. Mitral annular calcification is more likely to cause mitral regurgitation than stenosis. Two reasons could contribute to this. The normal sphincter like mechanism of mitral annular c...

Terbutaline and Albuterol for Lowering of Plasma Postassium
CONCLUSION: We conclude that in patients with terminal renal failure undergoing maintenance hemodialysis, intravenous bicarbonate is ineffective in lowering plasma potassium rapidly, and epinephrine is effective in only half the patients, whereas insulin in glucose is a fast and reliable form of therapy for hyperkalemic emergencies. Plasma aldosterone levels are appropriate in relationship to plasma potassium levels, and levels of other potassium-influencing hormones are generally normal. You Call That Efficacy? FDA Warns Amgen For Aranesp Promotion
For the past dozen years, Amgen has been marketing Aranesp to treat anemia caused by chronic kidney disease. So by now, one might expect the biotech to know how to successfully compile promotional material that is distributed to physicians. But apparently not, at least according to the FDA, which last week sent a letter chastising Amgen for committing some basic mistakes. What went wrong? The Amgen mailer omitted important risk information, suggested that Aranesp is useful in a broader range of conditions or patients than has been demonstrated by evidence and made unsubstantiated efficacy claims, according to the November ...

No Value For Renal-Artery Stenting In CORAL
Previous small studies have failed to find any benefit associated with renal-artery stenting, but the trials have been small and were not powered for clinical outcomes. Now, the CORAL (Cardiovascular Outcomes in Renal Atherosclerotic Lesions) trial, presented at the American Heart Association meeting and published simultaneously in the New England Journal of Medicine, offers strong and persuasive evidence that renal-artery stenting is not beneficial. A total of 947 patients with renal-artery stenosis and either systolic hypertension, despite taking two or more antihypertensive agents, or chronic kidney disease were ra...
