Cardiogenic shock and chronic kidney disease: Dangerous liaisons
CONCLUSIONS: Cardiogenic shock and CKD are frequent "cross-talking" conditions with limited therapeutic options, resulting in higher rates of death at 1month and 1year. RRT is a strong predictor of death, regardless of preexisting CKD. Multidisciplinary teams involving cardiac and kidney physicians are required to provide integrated care for patients with failure of both organs.PMID:38594150 | DOI:10.1016/j.acvd.2024.01.006 (Source: Archives of Cardiovascular Diseases)

Implantation of short-term biventricular assist device (BiVAD) using the CentriMag ™ system: the Manchester technique
AbstractBiventricular assist devices (BiVADs) using the CentriMag ™ system are being used increasingly as a form of short-term mechanical circulatory support for the treatment of acute cardiogenic shock from any aetiology. They can be used as a bridge to decision, recovery or transplantation. BiVADs are associated with better clinical outcomes when compared to v eno-arterial (VA) extracorporeal membrane oxygenator (ECMO) systems. In this paper, we describe a safe and reproducible method of BiVAD implantation using the CentriMag™ system at our institution. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

Characteristics of High-Performing Hospitals in Cardiogenic Shock Following Acute Myocardial Infarction
Cardiogenic shock (CS) remains a severe complication of acute myocardial infarction (AMI), carrying an in-hospital mortality of 30 to 50%.1-3 Despite more frequent percutaneous coronary intervention (PCI) and an array of mechanical circulatory support (MCS), the substantial mortality of AMI-CS nevertheless persists.1,4-7 Adherence to process measures in other cardiology domains, such as ejection fraction assessment after AMI or initiation of guideline-directed therapy in heart failure (HF), is associated with decreased mortality. Endothelial-Derived Microparticles Associate with Hospital Major Adverse Cardiovascular Events but not with Long-Term Adverse Events in Acute Myocardial Infarction
In conclusion, higher on-admission EDMP counts were independently associated with hospital MACE among AMI. However, on-admission and 30-day postdischarge EDMP and their changes did not impact long-term follow-up MACE. The management of heart failure cardiogenic shock: an international RAND appropriateness panel

From Blunt Cardiac Injury to Heart Transplant Following Motorcycle Collision
We present a case of a healthy 24-year-old man without significant past medical history who was in a motorcycle accident, resulting in sternal fracture and BCI. He was ultimately found to have thrombotic occlusion and dissection of his left anterior descending artery (LAD), requiring mechanical thrombectomy and drug-eluting stent, as well as subsequent hospitalizations and operations due to various complications. It was suspected that he went into ventricular fibrillation and had a second motorcycle collision, resulting in cardiogenic shock. Ultimately, his progression of ischemic cardiomyopathy and mitral regurgitation le...

Total Artificial Heart as Bridge to Transplant in a Patient with Cardiogenic Shock, Biventricular Thrombi, and Multisystem Thromboembolic Events
Introduction: Patients with cardiogenic shock and multisystem thromboembolic events have a high mortality and are at increased risk of complications despite use of advanced heart failure therapies including temporary and durable mechanical circulatory support and orthotopic heart transplantation (OHT). Daratumumab Monotherapy in Combination with VA ECMO as a Bridge to Heart Transplantation for Al Amyloid Cardiomyopathy
Introduction: AL amyloidosis is a systemic disease that can affect multiple different organ systems. Cardiac involvement is common and carries a high mortality rate. Traditional chemotherapy regimens are not well tolerated in the setting of decompensated heart failure. Monotherapy with daratumumab has shown efficacy in reducing light chain production. The physiologic and anatomical characteristics of cardiac amyloidosis pose significant challenges when considering temporary circulatory support for patients presenting with cardiogenic shock. Outcomes of Patients with Peripartum Cardiomyopathy Requiring LVAD Support: A Single-Center Experience
Purpose: Peripartum cardiomyopathy (PPCM) typically presents with left ventricular (LV) dysfunction at the end of pregnancy or after delivery. Rarely, PPCM leads to advanced heart failure and refractory cardiogenic shock. Durable left ventricular assist devices (dLVAD) are a feasible treatment option in these cases. In this retrospective review, we aim to investigate outcomes of PPCM patients on dLVAD support. (Source: The Journal of Heart and Lung Transplantation)
