Daratumumab Monotherapy in Combination with VA ECMO as a Bridge to Heart Transplantation for Al Amyloid Cardiomyopathy

Introduction: AL amyloidosis is a systemic disease that can affect multiple different organ systems. Cardiac involvement is common and carries a high mortality rate. Traditional chemotherapy regimens are not well tolerated in the setting of decompensated heart failure. Monotherapy with daratumumab has shown efficacy in reducing light chain production. The physiologic and anatomical characteristics of cardiac amyloidosis pose significant challenges when considering temporary circulatory support for patients presenting with cardiogenic shock.
