Job as General surgeon in critical access hospital
I am entering my fifth year of general surgery residency and have started looking for jobs for next year. I have always planned to end up in either a rural area or smaller city. A lot of the more heavily advertised jobs in these areas are for small critical access hospitals in rural areas. They typically have more frequent call and relatively high compensation. Kyphoplasty during fellowship
If one were to do a pain fellowship at an ACGME accredited program in a big city (NYC or LA for example), would kyphoplasty be part of the training? Its hard to tell as it seems like a significant portion of pain physicians do not perform them or at least do not advertise whether they perform them. (Source: Student Doctor Network)

Where would you go to earn the most?
If location, family and other factors were not an issue where would you move to in the US for the highest salary for 2-3 years? I am trying to pin down a location, so far Alaska seems to be the highest salaries I've seen advertised. Do $700k + jobs exist out there?
