70-Year-Old Man With Chronic Nausea and Vomiting
A 70-year-old man with a medical history notable for gastroesophageal reflux disease, achalasia (status post Nissen fundoplication take down and hiatal hernia repair with later takedown and Heller myotomy with Dor fundoplication), chronic obstructive pulmonary disease, heart failure with preserved ejection fraction, chronic back pain, osteoarthritis, opioid use, insulin-dependent diabetes, diabetic neuropathy, and a diagnosis of gastroparesis presented with symptoms of altered mental status in the setting of intractable nausea and vomiting.
