Anaplastic Large Cell Lymphoma (ALK-negative) related Heart Failure and Recurrence After Heart Transplantation

A 51-year-old woman diagnosed with systolic heart failure 6 months prior, presented in fulminant cardiogenic shock and required insertion of an intra-aortic balloon pump. Echocardiography showed severe biventricular failure with her left ventricular systolic function declining to 20% and biopsy of the right ventricular septum showed lymphocytic myocarditis. (Figure 1, A) Viral, fungal, parasitic, amyloid, and iron stains on the heart were negative. She was treated with corticosteroids but remained dependent on mechanical support leading to expedited heart transplant candidacy evaluation.
