Non-alcoholic fatty liver disease (NAFLD) and cardiovascular disease (CVD) are closely associated, but it is not clear whether this is simply the result of them sharing the same risk factors, e.g. obesity, dyslipidaemia, insulin resistance and type 2 diabetes. Jiang et al. propose a novel mechanism linking NAFLD and atherosclerosis. Using a series of advanced in vitro experiments, they show that steatotic hepatocytes release extracellular vesicles that contain microRNA-1. These extracellular vesicles induce a pro-inflammatory phenotype in endothelial cells tha t promotes atherogenic vessel activation.
