Study shows insufficient decrease in wound complications with sutured versus stapled skin closure in gastrointestinal operations
Commentary on: Tsujinaka T, Yamamoto K, Fujita J, et al.. Subcuticular sutures versus staples for skin closure after open gastrointestinal surgery: a phase 3, multicentre, open-label, randomised controlled trial. Lancet 2013;382:1105–12. Context Postoperative wound complications can occur in 9–26% of gastrointestinal operations.1–3 Skin closure with subcuticular sutures in clean operations, such as caesarean sections, are associated with decreased wound complications and increased patient satisfaction when compared with skin closure using staples.4 5 Whether these results are applicable to clean–con...

Is Intracranial Arterial Involvement A Subgroup of Neuro-Behcet Syndrome? (P5.035)
Conclusion:Neurological involvement in BS patients is generally due to venous inflammation. Intracranial arterial involvement in NBS is rare, but it should be kept in mind that it can occur during the course of the disease independent of other stroke risk factors.Disclosure: Dr. Zeydan has nothing to disclose. Paroxysmal Dysarthria and Ataxia after Treatment of Brainstem Encephalitis (P6.053)
CONCLUSIONS: Paroxysmal dysarthria and ataxia is most often recognized as a complication of multiple sclerosis, but it can occur in other neurologic diseases with midbrain lesions near or involving the red nucleus. Effective treatments include carbamazepine, acetazolamide, lamotrigine, and phenytoin.Study Supported by: N/A.Disclosure: Dr. Klaas has nothing to disclose. Dr. Boes has nothing to disclose. Factors Associated with Major Cardiovascular Events in Patients with Systemic Necrotizing Vasculitides: Results of a Longterm Followup Study.
CONCLUSION: These results suggest that factors associated with a higher MCVE risk in patients with SNV are NCEP/ATP III-defined high-risk status and BMI > 30 kg/m(2). Carotid IMT could help identify patients with SNV at risk of early MCVE. PMID: 24584925 [PubMed - as supplied by publisher]

Targeting Microglial Activation in Stroke Therapy: Pharmacological Tools and Gender Effects.
Abstract Ischemic stroke is caused by critical reductions in blood flow to a part of brain or spinal cord. Microglia are the resident immune cells of the central nervous system, and they respond to stroke by assuming an activated phenotype that releases cytotoxic cytokines, reactive oxygen species, proteases, and other factors. This acute, innate immune response may be teleologically adapted to limit infection, but in stroke this response can exacerbate injury by further damaging or killing nearby neurons and other cell types and by recruiting infiltration of circulating cytotoxic immune cells. The microglial resp...

Mortality of bullous pemphigoid in Singapore: risk factors and causes of death in 359 patients seen at the National Skin Centre.
CONCLUSION: This study confirmed an increased 3-year mortality rate for BP patients in Singapore. Risk factors for increased mortality include medical co-morbidities, especially neurological, cardiac and renal diseases. Treatment with combination therapy, including the use of low to moderate dose corticosteroid, appeared to decrease mortality risk in BP patients. This article is protected by copyright. C-reactive protein and fracture risk: European Prospective Investigation into Cancer Norfolk Study
Conclusion: A U-shaped association was observed between CRP and fracture risk. The increased risk of fracture observed at lower end of CRP compared to intermediate levels require further exploration, confirmation in other populations, and investigation into potential biological mechanisms.Highlights:

Internal carotid thrombus in patients with inflammatory bowel disease: Two cases.
We report 2 cases of ischemic strokes due to internal carotid thrombus in patients during active phase of IBD. Ulceration of early atherosclerotic plaques activated by a hypercoagulation state may cause a thrombus. A combined therapy with heparin and corticosteroids was used for both our patients. Lysis of the thrombus was obtained after several days without surgical treatment and shown by ultrasonography. These cases highlight an aetiology of stroke in patients with IBD and use of a synergic treatment to respond to hypercoagulability in link with IBD. Benefits and safety of this therapy should be confirmed with clinical s...
