Risk of Adverse Maternal and Fetal Outcomes during Pregnancy in Living Kidney Donors: A Matched Cohort Study
We examined the risk of adverse pregnancy outcomes in primiparous kidney donors compared to matched controls.MethodsFifty ‐nine women with a history of kidney donation prior to their first pregnancy with normal renal function and no history of kidney disease, diabetes or chronic hypertension were matched 1:4 by age (within 2 years) and race to women with two kidneys using data from an integrated healthcare delivery s ystem. Adverse pregnancy outcomes were defined as preterm delivery (delivery 3 days, infant death/transfer to acute facility and low birth weight (

The fat mass and obesity related gene polymorphism influences the risk of rejection in heart transplant patients
AbstractHeart transplantation is a relatively common treatment for end ‐stage heart failure. The major complication of heart transplantation is organ rejection. Epigenetic could play a role in the pathogenesis of organ rejection, and theFTO gene is a mediator of DNA methylation. We analyzed a taggingFTO SNP rs17817449 in both donor and recipient DNA obtained through 370 heart transplantations. RecipientFTO genotypes were not associated with either type of rejection or with the general increase in the risk of rejection. When compared with patients without a history of rejection, carriers of transplanted hearts with theFTO...

Robot ‐Assisted Laparoscopic Versus Laparoscopic Donor Nephrectomy in Renal Transplantation: A Meta‐Analysis
AbstractBackgroundCurrently, the robot ‐assisted laparoscopic donor nephrectomy (RDN) technique is used for live donor nephrectomy. Does it provide sufficient safety and benefits for living donors? We conducted a meta‐analysis to assess the safety and efficacy of RDN compared with the laparoscopic donor nephrectomy (LDN).Material and methodsEligible studies were retrieved and screened from electronic databases from 1999 onward: PubMed, Cochrane Library and Web of Science. Relevant parameters were explored using Review Manager V5.3 and included operative time, warm ischemia time, estimated blood loss, and length of hosp...

Implementation of a transition model to adult care may not be enough to improve results: National study of kidney transplant recipients
AbstractAdolescents with a kidney transplant (KT) require special attention during the transition of care. Few longitudinal studies have assessed the effect of transition models (TM) on patient outcomes. Between 1986 and 2013, 239 pediatric patients underwent KT in Finland, of whom 132 have been transferred to adult care. In 2005, a TM was developed following international recommendations. We compared patient (PS) and graft survival (GS) rates before and after the introduction of the TM. PS and GS at 10 years were similar before and after the implementation of the TM (PS 85% and 90% respectively, P=0.626; GS 60% and 58%, r...

The impact of blood pressure variability and pulse pressure on graft survival and mortality after kidney transplantation
Conclusion: Visit‐to‐visit blood pressure variability is not associated with mortality or graft loss after kidney transplantation in this retrospective analysis. In analogy to the general population, however, there is an inverse relationship of survival and pulse pressure as a marker of arterial stiffness. This article is protected by copyright. Once ‐daily vs twice‐daily tacrolimus for de novo living kidney transplantation patients including ABO/HLA compatible and incompatible: A randomized trial
AbstractTacrolimus (TAC) is available as a twice ‐daily capsule (TAC‐BID), once‐daily capsule (TAC‐QD), and once‐daily tablet. Recipients with ABO‐incompatible/anti‐human leukocyte antigen (HLA)‐incompatible transplantation were excluded in previous trials and have thus not been evaluated. We conducted a 5‐year trial to determine whether TAC‐QD is noninferior to TAC‐BID for transplant outcomes. Adults who underwent de novo living kidney transplantation were randomly assigned (62 TAC‐QD; 63 TAC‐BID). We did not exclude ABO‐/HLA‐ incompatible transplantation. TAC was initiated 7 days preope...
