Quantifying Selection Bias in National Institute of Health Stroke Scale Data Documented in an Acute Stroke Registry Original Articles
Conclusions— We demonstrate modest, but important, selection bias in documented NIHSS data, which are missing more often in patients with less severe stroke. The population mean NIHSS score was overestimated by >2 points, which could significantly alter the risk profile of hospitals treating patients with ischemic stroke and subsequent hospital risk–adjusted outcomes.

Prehospital Phase of the Stroke Chain of Survival: A Prospective Observational Study Stroke
Conclusions Fast emergency medical services activation and ambulance transport promoted early hospital arrival and treatment. Although patient-dependent delays still dominate the prehospital process, it should be ensured that the minutes on the scene are well spent.

Impact of Increased Early Statin Administration on Ischemic Stroke Outcomes: A Multicenter Electronic Medical Record Intervention Stroke
Conclusions A simple EMR change increased early statin administration in ischemic stroke and was associated with improved clinical outcomes. This is, to our knowledge, the first EMR intervention study to show that a modification of an electronic order set resulted in improved clinical outcomes.

Regional Learning Collaboratives Produce Rapid and Sustainable Improvements in Stroke Thrombolysis Times Original Articles
Conclusions— Using a learning collaborative model at Chicago’s 15 primary stroke centers, we observed major reductions in DTN times within 1 quarter of implementation. Regional collaboration and best practices sharing should be a model for rapid and sustainable system-wide quality improvement.

Left ventricular ejection fraction and aortic stiffness are independent predictors of neurological outcome in acute ischemic stroke
Conclusion: These findings provide evidence that in ischemic stroke, preserved ejection fraction and lower pulse wave velocity are associated with early favorable outcome, independently of other known prognostic factors, including blood pressure.

Healthcare Resource Availability, Quality of Care, and Acute Ischemic Stroke Outcomes Health Services and Outcomes Research
ConclusionsSignificant variation exists in regional availability of healthcare resources for acute ischemic stroke treatment, yet among Get With the Guidelines‐Stroke hospitals, quality of care and in‐hospital outcomes did not differ by regional resource availability.

Post-Glucose Load Measures of Insulin Resistance and Prognosis of Nondiabetic Patients With Ischemic Stroke Stroke
This study aimed to estimate the association between post–glucose load measures of insulin resistance and prognosis of nondiabetic patients with ischemic stroke. Methods and ResultsData were derived from the ACROSS‐China (Abnormal Glucose Regulation in Patients with Acute Stroke across China) registry. Patients with ischemic stroke without a history of diabetes mellitus were included. Two post–glucose load measures of insulin sensitivity, the insulin sensitivity indices ISI(composite) and the ISI0,120, were calculated. Outcomes included stroke recurrence, all‐cause death, and poor functional outcome at 12 months. A...

Ethnic Differences in Prevalence of Post-stroke Depression Original Articles
Conclusions: MAs had a higher prevalence of PSD at 90 days than NHWs. The ethnic difference was explained by sociodemographic and health factors, especially low educational attainment.

The Case for Selective Patent Foramen Ovale Closure After Cryptogenic Stroke Advances in Interventional Cardiology
Controversy has persisted for over a decade whether transcatheter patent foramen ovale (PFO) closure reduces the rate of recurrent ischemic stroke for patients who have had a cryptogenic ischemic stroke and have a PFO. In September 2017, 3 positive randomized trials: RESPECT long-term (Recurrent Stroke Comparing PFO Closure to Established Current Standard of Care Treatment trial), REDUCE (GORE® HELEX® Septal Occluder / GORE® CARDIOFORM Septal Occluder and Antiplatelet Medical Management for Reduction of Recurrent Stroke or Imaging-Confirmed TIA in Patients With Patent Foramen Ovale [PFO]), and CLOSE (Patent For...

CAABL-AF (California Study of Ablation for Atrial Fibrillation): Mortality and Stroke, 2005 to 2013 Original Articles
Conclusions: In this large population-based study of hospitalized patients with nonvalvular AF, ablation was associated with lower mortality, ischemic stroke, and hemorrhagic stroke compared with controls.

Yield of Dual Therapy With Statin and Ezetimibe in the Treat Stroke to Target Trial
CONCLUSIONS: In the TST trial (Treat Stroke to Target), targeting an LDL cholesterol of < 70 mg/dL with a combination of statin and ezetimibe compared with 100±10 mg/dL consistently reduced the risk of subsequent stroke.REGISTRATION: URL: https://www.CLINICALTRIALS: gov; Unique identifier: NCT01252875. Joint Commission Primary Stroke Centers Utilize More rt-PA in the Nationwide Inpatient Sample Stroke
Conclusions Subjects evaluated at PSCs were more likely to receive rt-PA than those evaluated at non-PSCs. This association was significant after adjustment for patient and hospital-level variables. Systems of care are necessary to ensure stroke patients have rapid access to PSCs throughout the United States.

Abstract 7: Reduced Hospitalization For Transient Ischemic Attack In The United States Over The Past Decade Concurrent I Session B: Oral Abstract Presentations on Stroke Topics
CONCLUSION: TIA hospitalizations in the United States decreased from 2000 to 2010 as the number of ischemic stroke hospitalizations increased. This may be a result of increased imaging utilization and use of tissue-based diagnosis of TIA. Cases of cerebral ischemia with resolved symptoms coded as TIA in the past may be coded as ischemic stroke with MRI imaging evidence of brain injury, yet further study is needed to confirm this finding.

Association of Body Mass Index and Mortality After Acute Ischemic Stroke Original Articles
Conclusions— Severe obesity is associated with increased poststroke mortality in middle-aged and older adults. Stroke patients with class 2 obesity had the lowest mortality risk. More research is needed to determine weight management goals among stroke survivors.

Cost-Effectiveness of Clopidogrel-Aspirin Versus Aspirin Alone for Acute Transient Ischemic Attack and Minor Stroke Stroke
Conclusions Early 90-day clopidogrel-aspirin regimen for acute TIA or minor stroke is highly cost-effective in China. Although clopidogrel is generic, Plavix is brand in China. If Plavix were generic, treatment with clopidogrel-aspirin would have been cost saving.
