Predictive and individualized management of stroke —success story in Czech Republic
AbstractThe model of centralized stroke care in the Czech Republic was created in 2010 –2012 by Ministry of Health (MH) in cooperation with professional organization—Cerebrovascular Section of the Czech Neurological Society (CSCNS). It defines priorities of stroke care, stroke centers, triage of suspected stroke patients, stroke care quality indicators, their monitoring, and repor ting. Thirteen complex cerebrovascular centers (CCC) provide sophisticated stroke care, including intravenous thrombolysis (IVT), mechanical thrombectomy (MTE), as well as other endovascular (stenting, coiling) and neurosurgical procedures. T...

China stroke surveillance report 2021
AbstractSince 2015, stroke has become the leading cause of death and disability in China, posing a significant threat to the health of its citizens as a major chronic non-communicable disease. According to the China Stroke High-risk Population Screening and Intervention Program, an estimated 17.8 million [95% confidence interval (CI) 17.6 –18.0 million] adults in China had experienced a stroke in 2020, with 3.4 million (95% CI 3.3–3.5 million) experiencing their first-ever stroke and another 2.3 million (95% CI 2.2–2.4 million) dying as a result. Additionally, approximately 12.5% (95% CI 12.4–12.5%) of stroke survi...

Stroke: the critically neglected first year post-stroke
Journal of Integrated Care, Ahead of Print. Purpose Stroke is a leading cause of death and disability in the USA and worldwide. While stroke care has evolved dramatically, many new acute approaches to therapy focus only on the first 3-12 hours. Significant treatment opportunities beyond the first 12 hours can play a major role in improving outcomes for stroke patients. The purpose of this paper is to highlight the issues that affect stroke care delivery for patients and caregivers and describe an integrated care model that can improve care across the continuum. Design/methodology/approach This paper details evidence-based ...

The Impact Of Stroke Centers On The Length Of Stay Among Ischechemic Stroke Patients In Alberta
Although previous studies have shown that stroke centers reduce mortality in patients with ischemic stroke, the association between stroke centers and reduction in length of stay (LOS) in hospital has not been demonstrated. We sought to evaluate the impact of stroke centres as designated by the Alberta Provincial Stroke Strategy (APSS) in Alberta on the LOS of acute ischemic stroke patients.

The influence of the day of the week of hospital admission on the prognosis of stroke patients.
This study aimed to evaluate the weekday and weekend distribution of stroke case hospital admissions and their respective prognosis based on a sample from the Estudo de Mortalidade e Morbidade do Acidente Vascular Cerebral (EMMA), a cohort of stroke patients admitted to a community hospital in the city of São Paulo, Brazil. We ascertained all consecutive cases of first-time strokes between April 2006 and December 2008 and performed a subsequent one-year follow-up. No association was found between frequency of hospital admissions due to ischemic and hemorrhagic strokes and the specific day of the week on which the admissio...

Analysis of barriers to the use of thrombolytics in ischemic stroke in a private hospital in Rio de Janeiro, Brazil.
The objective was to discuss barriers to thrombolytic therapy in a private hospital. This was a retrospective cohort study of patients with symptoms of acute ischemic stroke entering the emergency department of a private hospital in Rio de Janeiro, Brazil, from 2009 to 2011. 257 patients were admitted with suspected ischemic stroke. Among the 156 patients with confirmed diagnosis of ischemic stroke, 18 (11.5%) received thrombolytic therapy. Of the 30 patients with ischemic stroke within the therapeutic window and meeting NIHSS criteria for thrombolysis, 20 were not thrombolysed. Nine of these were due to administrative bar...

Risk factors of hemorrhagic and ischemic stroke among hospitalized patients in Bangladesh - A case control study
The risk factor profiles, management and outcome have significant difference between stroke subsets. Aim of this study was to investigate the risk for the two most common subtypes of stroke in Bangladeshi population. Seventy cases of hemorrhagic stroke (HS) and 105 cases of confirmed ischemic stroke (IS) were recruited from the Shaheed Suhrawardy Medical College Hospital (ShSMCH) and Dhaka Medical College Hospital between January-June 2011. Total 171 age, sex matched controls were selected from the hospitalized patients with history of no stroke ever. Average hemorrhagic stroke patients (60.4 ±12.3 years) were younger tha...

Care Coordination for Community Transitions for Individuals Post-stroke Returning to Low-Resource Rural Communities
AbstractHigh rates of hospital readmissions have been shown within 12 months post-discharge from inpatient rehabilitation following stroke. Multiple studies coupled with our previous work indicate a need for care support for stroke survivors ’ transitions to the community. The Kentucky Care Coordination for Community Transitions (KC3T) program was developed to provide access to medical, social, and environmental services to support community transitions for individuals with neurological conditions and their caregivers living in Kentucky. This program assessment was conducted to determine the effectiveness of using a spec...

EE47 Short-Term Cost-Effectiveness of Using Mobile Stroke Unit VS. Standard Ambulance for Patients with Suspected Stroke
Mobile stroke units (MSUs) have the potential to improve time-sensitive stroke care delivery by appropriate triage of patients with large-vessel-occlusion (LVO) to comprehensive stroke centers (CSCs) for endovascular treatment (EVT) and non-LVO patients to primary stroke centers (PSCs). The purpose of this study was to determine the cost-effectiveness of MSU when compared to the standard ambulance (SA) transport of suspected stroke patients to the appropriate hospital.

Implementing Thrombolytic Guidelines in Stroke Care: Perceived Facilitators and Barriers
We performed a qualitative study to identify facilitators of and barriers to the implementation of national guidelines on thrombolytic therapy for acute ischemic stroke. We interviewed physicians and nurses at nine Swedish hospitals using 16 explorative, semistructured interviews, and selected hospitals based on their implementation rate of new stroke care methods according to data from the Swedish Stroke Register, Riks-Stroke. Through content analysis, we identified facilitators and barriers to implementation, which we classified into three categories: (a) individuals, (b) social interactions and context, and (c) organiza...
