Prediction of Occurrence of Cerebral Infarction After Successful Mechanical Thrombectomy for Ischemic Stroke in the Anterior Circulation by Arterial Spin Labeling
ConclusionThe AI of ASL-CBF before MT or a  combination of a history of atrial fibrillation, the AI of ASL-CBF before MT, and the time from onset to reperfusion can be used to predict the occurrence of infarction in patients arriving within 8 h after stroke onset in which reperfusion with MT was successful.

Myocardial Infarction Bends the Curve of Age-Related Cognitive Decline
In this issue of JAMA Neurology, Johansen et al reported that there was an acceleration of age-related cognitive decline following acute myocardial infarction (MI). In an analysis of more than 30  000 persons pooled from 6 major US epidemiological cohort studies, the 1033 individuals who had an MI during study follow-up had a steeper annual decline in cognition after the MI compared with before the MI. As expected, all persons with or without MI had a small age-related annual decline in co gnition. This age dependence is a feature of most neuropsychological tests. However, after MI, the annual rate of decline accelerated...

An elderly case with late carotid stent thrombosis: possible role of antiphospholipid antibodies
The case study speculates that the antiphospholipid antibodies acquired during the follow-up period of carotid artery stenting may cause late stent thrombosis that is resistant to direct oral anticoagulants. A 73-year-old man was hospitalized with complaints of weakness in the right lower extremity. The patient had undergone carotid artery stenting for symptomatic stenosis of the left internal carotid artery 6 years prior and had received antiplatelet therapy with clopidogrel 75  mg/day. As the patient had developed atrial fibrillation without stent stenosis at the age of 70 years, anticoagulation therapy with rivaroxaban...

Embolic stroke and atrioesophageal fistula as complications of atrial ablation
We present a case of a patient with cardioembolic cerebral infarcts and sepsis secondary to an atrioesophageal fistula, which likely developed after an atrial ablation procedure for atrial fibrillation.

Patent foramen ovale closure in non-elderly and elderly patients with cryptogenic stroke: a hospital-based cohort study
DiscussionPFO closure was associated with a reduced risk of the primary outcome in patients with PFO and CS in our total cohort, which included non-elderly and elderly patients. Compared to those without PFO closure, elderly patients with PFO closure had a better functional outcome at 180 days. PFO closure might be considered in selected elderly patients with PFO.

Effect of Intravenous Thrombolytic Dose of Alteplase on Long-Term Prognosis in Patients with Acute Ischemic Stroke
ConclusionsStandard-dose alteplase treatment reduced the risk of long-term recurrence of AIS after hospital discharge and the alteplase dose was not associated with the long-term risk of death from AIS.

Prediction of underlying atrial fibrillation in patients with a cryptogenic stroke: results from the NOR-FIB Study
ConclusionClinical risk scores may guide a personalized evaluation approach in CS patients. Increasing awareness of the usage of available AF-predicting scores may optimize the arrhythmia detection pathway in stroke units.

Clinical features of ischemic stroke in patients with nonvalvular atrial fibrillation combined with intracranial atherosclerotic stenosis
ConclusionNVAF with ICAS ischemic stroke is more likely to occur in older patients. Infarctions occurred mainly in the right cerebral hemisphere. Neurological deficits in NVAF are more severe than those in NVAF combined with ICAS and in simple ICAS ischemic strokes. HsCRP, LEVF, andNTpro-BNP seem to be closely associated with NVAF+ICAS ischemic stroke.

Thrombectomy with or without Bridging Thrombolysis for Anterior Circulation Stroke
Conclusions: In this study, BT seems to yield better clinical and procedural outcomes with lower complication rates than d-MT. These findings may support the additional value of intravenous alteplase in anterior system strokes. Further large-scale, prospective, randomized-controlled studies will clarify the gray lines in this consensus, but this paper is important for reflecting the real-world data in developing countries.

Increased Serum EphrinA1 Is Associated With Parenchymal Hematoma (PH) After Ischemic Stroke
Conclusion Higher serum EphrinA1 is independently associated with higher risk of PH after ischemic stroke. Future studies with larger sa mple size are needed to validate our findings and elucidate the potential role of EphrinA1 in PH.

Cryptogenic stroke as a working diagnosis: the need for an early and comprehensive diagnostic work-up
