Extracranial-Intracranial Bypass Surgery for Stroke Prevention
The concept of performing an extracranial-intracranial (EC-IC) arterial bypass operation for a patient with an internal carotid artery (ICA) or middle cerebral artery (MCA) occlusion is intuitively appealing. In these patients, it is believed that areas of hypoperfusion exist and anastomosis of the superficial temporal artery to the MCA is designed to augment brain perfusion. In vascular territories outside of the brain, bypass procedures performed to augment arterial flow typically improve clinical outcomes. For reasons unknown, however, the brain appears to respond differently to bypass surgery.

Extracranial-Intracranial Bypass and Risk of Stroke and Death in Patients With Symptomatic Artery Occlusion
This clinical trial compares the efficacy of extracranial-intracranial bypass surgery plus medical therapy vs medical therapy alone in symptomatic patients with internal carotid artery or middle cerebral artery occlusion and hemodynamic insufficiency on a composite of stroke or death within 30 days or ipsilateral ischemic stroke beyond 30 days through 2 years after randomization.

Consensus statement for the management of incidentally found brain white matter hyperintensities in general medical practice
Med J Aust. 2023 Aug 21. doi: 10.5694/mja2.52079. Online ahead of print.ABSTRACTINTRODUCTION: There is a paradigm shift in our understanding of white matter hyperintensities (WMH) found on brain imaging. They were once thought to be a normal phenomenon of ageing and, therefore, warranted no further investigation. However, evidence now suggests these lesions are markers of poor brain and cardiovascular health, portending an increased risk of stroke, cognitive decline, depression and death. Nevertheless, no specific guidelines exist for the management of incidentally found WMH for general medical practitioners and other clin...

Effect of teleconsultation on the application of thrombolytic therapy in stroke patients in the emergency department
ConclusionsTeleconsultation was a successful and reliable strategy in assessing patients with ischemic stroke and making decisions for IV-tPA. Moreover, patient assessment via teleconsultation was less time consuming. The results of the study are promising for the use of teleconsultation in the future.

Is a novel digital system for arm and hand rehabilitation suitable for stroke survivors? A qualitative process evaluation of OnTrack
Conclusions This process evaluation supported initial theoretical assumptions that OnTrack would enable activity practice through the use of remote monitoring. There was a strong emphasis on the coaching role as a mechanism of impact supporting the technological intervention. These findings will inform the next stages of delivery in a definitive trial. Trial registration number NCT03944486.

Screening over 100 000 patients in 39 general practices in the Netherlands for anticoagulation underprescription in atrial fibrillation: a descriptive, cross-sectional study
Conclusions In this large Dutch study among GPs, we observed 9.8% underprescription of OAC in AF patients. In 76% of the AF patients lacking a prescription for OAC, no documentation for deviating from the guidelines was found. Only in a minority of cases detection of OAC underprescription lead to OAC initiation.

Relapse of nephrotic syndrome with unusual thromboembolic event: A case report
Key Clinical MessageMost children with nephrotic syndrome heal without any sequelae. However, rare life-threatening complications such as thromboembolism may occur in pediatric nephrotic syndrome and should be considered in those with a new-onset neurologic deficit.AbstractThe thromboembolism (TE) as a complication of nephrotic syndrome (NS) is rare and serious, and may involve renal, cerebral, pulmonary, or peripheral venous and/or arterial thrombosis. Here, we describe a 4.5-year-old male with a history of nephrotic syndrome, who developed hemorrhagic stroke in the territory of middle cerebral artery (MCA).

Effectiveness of acceptance and commitment therapy in patients who had a stroke: a systematic review protocol
This study aimed to systematically evaluate the intervention effect of ACT in patients who had a stroke, which may provide further clinical evidence. Methods and analysis A systematic search of databases, including CNKI, WanFang Data, VIP, CBM, PubMed, Embase, Cochrane Library, CINAHL and APA PsycArticles, will be conducted from their inception to 31 October 2022. In-thrombus thrombin secretion: a new diagnostic marker of atrial fibrillation in cryptogenic stroke
Isr Med Assoc J. 2022 Dec 1;25(12):853-854.ABSTRACTEndovascularly retrieved clots may be a potential resource for diagnosing stroke etiology. This method may influence secondary prevention treatment. We measure thrombin activity eluted by serially washing clots. We concluded that an assay measuring the change in thrombin in clots retrieved during acute stroke endovascular thrombectomy procedures may serve as a diagnostic marker of the origin of the clot. The suggested mechanism for these differences may be the clot location before its retrieval, with high blood flow causing thrombin washout in atherosclerotic clots, in con...
