Vestibular Neuritis: Recent Advances in Etiology, Diagnostic Evaluation, and Treatment.
Authors: Le TN, Westerberg BD, Lea J Abstract Purpose of Chapter: This chapter highlights the recent advances in etiology, diagnostic evaluation, and management of vestibular neuritis (VN). Recent Findings: The viral hypothesis has been strengthened with new evidence as the main etiology of VN. Recent evidence indicates that bedside oculomotor findings play a critical role in differentiating VN from stroke. The implementation of cervical and ocular vestibular evoked myogenic potential, and video head impulse test in vestibular function testing has made it possible to diagnose selective damage of the vestibular nerv...

Risk of mortality and cardiovascular events following macrolide prescription in chronic rhinosinusitis patients: a cohort study using linked primary care electronic health records.
CONCLUSIONS: Although not statistically significant, our best estimates suggest an increased short-term risk of myocardial infarction in patients with CRS following macrolide prescription, supporting previous observational evidence. However, confounding by indication remains a possible explanation for this apparent increased risk. We found no evidence of longer term increased risks. Recurrent and Progressive Facial Baroparesis on Flying Relieved by Eustachian Tube Dilation.
CONCLUSIONS:: This study presents a rare case of facial baroparesis on commercial flight descent that resolved after left Eustachian tube dilation. Although unilateral facial palsy can be concerning for stroke, a history of ear fullness and pressure may suggest facial baroparesis instead. For recurrent and progressive cases, Eustachian tube dilation should be considered for treatment. PMID: 30895801 [PubMed - as supplied by publisher]

Severe Obstructive Sleep Apnea with Imaged Carotid Plaque Significantly Associated with Systemic Inflammation
Persons with obstructive sleep apnea (OSA) are at heightened risk of myocardial infarction (MI) and stroke caused by adiposity and intermittent hypoxia which provoke proinflammatory cytokines to induce systemic/vascular inflammation resulting in endothelial dysfunction and development of atherosclerotic plaque. We aim to compare the levels of systemic inflammation between groups of patients having severe OSA with and without carotid artery calcified plaque (CACP+/CACP –) on their panoramic image (PI) as indexed by the neutrophil-lymphocyte ratio (NLR).

Severe Obstructive Sleep Apnea With Imaged Carotid Plaque Is Significantly Associated With Systemic Inflammation
This study compared levels of systemic inflammation between groups of patients with severe OSA with and without carotid artery calcified plaque (CACP+ and CACP −, respectively) on their panoramic image (PI) as indexed by the neutrophil-to-lymphocyte ratio (NLR).

Perioperative Management For the Cocaine-Positive Patient Undergoing Elective Surgery Under General Anesthesia
Cocaine use in the United States is widespread and on the rise.1,2 Due to the increased prevalence of cocaine use, it is likely that the oral and maxillofacial surgeon (OMS) will come across multiple instances of cocaine-positive patients presenting for elective surgery. Studies have already shown that many oral and maxillofacial surgery cases are cancelled due to the presence of cocaine metabolites in urine tests.1,3 Cocaine is a known risk factor for cardiovascular complications (eg, stroke, myocardial infarction, dysrhythmia, angina, hypertension, tachycardia, prolonged QT interval, coronary vasospasm, and aortic dissec...

Sudden Sensorineural Hearing Loss in Hemodialysis Patients Could be a Marker of Pathogenic Progression in the Mortality and Atherosclerotic Events: A National Cohort Study
Conclusion: This study is the first to signify that the risks of mortality and atherosclerotic complications in HD patients with new-onset SSHL are increased. Therefore, we should pay attention to risks of mortality and atherosclerotic events in HD patients with new-onset SSHL.

Positive Association between Neutrophil-Lymphocyte Ratio and Presence of Panoramically Imaged Carotid Atheromas Among Men
Heightened levels of systemic inflammation documented by elevated neutrophil-lymphocyte ratios (NLRs) characterize robust atherosclerosis processes evidenced by carotid and coronary artery plaques on ultrasound and angiography with associated strokes and myocardial infarctions (MIs). Therefore, we sought to determine if calcified carotid artery plaques (CCAP) on panoramic images (PIs), known to herald future stroke and MI, are associated with elevated NLRs.

A Study on Etiopathogenesis of Vocal Cord Paresis and Palsy in a Tertiary Centre
AbstractTo identify patients of vocal cord paresis and palsy and to establish an etiological diagnosis based on a study performed in a tertiary centre. Study was done prospectively in the Department of ENT in KIMS Hospital, Bangalore, for 1  year, from June 2016 to June 2017. 100 patients with vocal cord paresis and palsy were identified and examined by using various tests and investigations to establish the etiology. Most of the patients presented with complaints of change in voice (92%). Some of the other common presenting complaint s included noisy breathing and difficulty in swallowing, difficulty in voice production ...

To Pack or Not to Pack: Inpatient Management of Epistaxis in the Elderly.
Conclusion Nonpacking interventions in the elderly do not appear to be associated with increased morbidity or mortality when compared to nasal packing only but appear to be associated with increased hospital charges and length of stay. Embolization in the elderly results in greater hospital charges but no change in outcome when compared to ligation. PMID: 30270635 [PubMed - as supplied by publisher]

Diagnosis and Treatment of Squamous Cell Carcinoma in Lower Lip: Case Report
A 67-year-old woman attended the stomatology clinic complaining of a wound on the lip, painful, with an evolution of 3 months. During this period, she was treated by a dermatologist for herpes simplex, but without resolution. Anamnesis indicated systemic arterial hypertension and previous stroke; she denied smoking or performing activities causing sun exposure. A physical examination revealed an exophytic ulcer on the lower lip with a crusted necrotic background, indurated rounded borders, soft base, 1.5  cm in size, and loss of the mucocutaneous line.
