Efficacy and safety of early parenteral anticoagulation as a bridge to warfarin after mechanical valve replacement.
In conclusion, we found that early after mechanical valve replacement, therapeutic dose bridging was associated with a similar risk of thromboembolic complications, but a 2.5 to 3-fold increased risk of major bleeding compared with prophylactic dose bridging. Stroke in Patients With Aortic Stenosis: The Simvastatin and Ezetimibe in Aortic Stenosis Study Clinical Sciences
Conclusions— In patients with aortic stenosis not prescribed oral anticoagulation, atrial fibrillation, AVR with concomitant coronary artery bypass grafting, and CHA2DS2-VASc score were the major predictors of stroke. Ten-year screening for thrombocytopenia after aortic valve replacement
CONCLUSIONS Thrombocytopenia and Elisa (+) are more common after AVR than after other procedures, and both were associated with increased adverse clinical outcomes. Age and lower preoperative platelet count were associated with postoperative thrombocytopenia and Elisa (+).

Mechanical heart valve prosthesis and warfarin – Treatment quality and prognosis
Conclusion: Patients with a lower warfarin treatment quality measured by TTR have a higher risk of complications such as severe bleeding or death. A TTR of 83% or higher at the individual level should be obtained for best outcome.

Conclusions: Patients on long-term support have an excellent haemodynamic profile and show an acceptable risk-benefit ratio. Outcome of redo surgical aortic valve replacement in patients ≥ 80 years: results from the multicenter RECORD initiative.
CONCLUSIONS: Octogenarians undergoing S-AVR after prior cardiac surgery have similar immediate postoperative outcome to younger patients and their 5-year outcome is excellent. Mitral valve repair and bioprosthetic replacement without postoperative anticoagulation does not increase the risk of stroke or mortality ADULT CARDIAC
CONCLUSIONS Despite current guidelines recommending postoperative anticoagulation following MVRR or bioprosthetic replacement, the avoidance of warfarin does not increase perioperative complications and has no impact on intermediate survival. Accordingly, a prospective randomized study to adjudicate the role of extended warfarin thromboprophylaxis in mitral valve surgery is warranted.
