Oral Anticoagulation.
CONCLUSION: The evidence base for anticoagulation over a time frame of several years is inadequate at present, and direct comparative data for the different types of NOAC are not yet available. PMID: 30602410 [PubMed - in process]

Aortic valve replacement by mechanical prosthesis in children: A positive long-term outcome including for patients with an infantile Marfan syndrome
ConclusionsLong-term results after AVR are excellent. Complications associated with surgery and long-term anticoagulation are rare. This study gives confidence in aortic mechanical prosthesis valve replacement including for children with an infantile MS.

Systemic atrioventricular valve replacement by mechanical prosthesis in children: Evolution in practice and predictors of long-term outcome
ConclusionsSAVR replacement remains a challenging procedure with high mortality but limited mortality related to anticoagulation. Modifications of the operative technics are one of the major causes of better long-term outcome.

Benefit of Four-Dimensional Computed Tomography Derived Ejection Fraction of the Left Atrial Appendage to Predict Thromboembolic Risk in the Patients with Valvular Heart Disease.
CONCLUSIONS: In the patients who underwent 4DCT for cardiac valve evaluation before surgery, EFCT by volume analysis might have additional role to evaluate LAA function and estimate the risk of thrombus. PMID: 30468031 [PubMed - as supplied by publisher]

Antithrombotic Strategies after Bioprosthetic Aortic Valve Replacement: A Systematic Review
ConclusionsTreatment with warfarin or aspirin both lead to similar outcomes after surgical bAVR. Combining aspirin with warfarin may lead to a small decrease in thromboembolism and mortality but is accompanied by increased bleeding. For TAVR patients, aspirin is equivalent to DAPT for reducing thromboembolism and mortality, with a possible decrease in bleeding.

Clinical Leaflet Thrombosis in Transcatheter and Surgical Bioprosthetic Aortic Valves by Four-Dimensional Computed Tomography
ConclusionsOne-third of patients with clinical or echocardiographic indications suggestive of leaflet thrombosis were found to have evidence of leaflet thrombosis using 4DCT. This allowed tailored anticoagulation therapy with resolution of the thrombus in most patients and avoiding unnecessary anticoagulation in the remaining two-thirds of patients.Visual Abstract

Immediate Improvement in Left Atrial Function After Transcatheter Aortic Valve Replacement on Doppler Echocardiography.
CONCLUSIONS: There is immediate improvement in LA function and an increase in LAA velocities after TAVR. This improvement may benefit hemodynamics immediately after TAVR, but may also increase the short-term stroke risk (as recently shown in two independent studies) in patients with LAA thrombus or low emptying velocities. Therefore, evaluation of LA function and LAA thrombus may be warranted to identify patients at high risk for periprocedural stroke and guide the need for anticoagulation therapy. DangasTranscatheter aortic valve implantation, also called transcatheter aortic valve replacement (TAVR), is the treatment of choice for patients with severe aortic stenosis and intermediate to high operative risk. A significant portion of TAVR patients have atrial fibrillation (AF) requiring chronic oral anticoagulation. In moderate...

Clinical Leaflet Thrombosis in Transcatheter and Surgical Bioprosthetic Aortic Valves by 4DCT
ConclusionsIn patients with clinical or echocardiographic indications suggestive of leaflet thrombosis, 1/3rd of patients were found to have evidence of leaflet thrombosis using 4DCT. This allowed tailored anticoagulation therapy with resolution of the thrombus in most patients avoiding unnecessary anticoagulation in the remaining 2/3rds of patients.

Systemic atrioventricular valve replacement by mechanical prosthesis in children: Evolution in practice and predictors of long-term outcome
ConclusionsEarly mortality after SAVR decreased during the second period of our study. Modification of the operative technique was the main cause of better early outcomes. SAVR replacement remains a challenging procedure with high mortality but limited mortality related to anticoagulation.

Long-standing persistent effects of atrial volume reduction combined with pulmonary vein isolation
Publication date: Available online 18 June 2018Source: Journal of Cardiology CasesAuthor(s): Masami Takagaki, Naoko Ikeda, Hiroki Yamaguchi, Shinichi Mitsuyama, Tasuku Kadowaki, Tatsuya NakaoAbstractAlthough surgical left atrial (LA) volume reduction combined with mitral valve surgery and/or surgical ablation for atrial fibrillation has been reported to be effective, its long-term outcomes in the absence of mitral procedure are not well established. A 74-year-old man with two previous sternotomies—the first for pericardiectomy due to constrictive pericarditis and the second for mitral valve replacement with mechanical va...

Fourth mitral valve replacement in a 15-year span in a woman of childbearing age
Publication date: Available online 13 June 2018Source: Journal of Cardiology CasesAuthor(s): Laura Leticia Rodríguez-Chávez, Grecia Iveth Maryelis Raymundo-Martínez, Rodrigo Gopar-NietoAbstractThere is no report of four valve replacement surgeries in a woman during 15-year follow-up. A 23-year-old female was diagnosed at 7 years with rheumatic fever. She developed severe mitral regurgitation and at the same age a prosthetic valve was placed. In the next 15 years her mitral valve was changed 3 times because of clinical, echocardiographic, and histopathologic evidence of thrombosis. Her coagulation tests all showed infrat...
