Procedure- Related and Perioperative Strokes in Patients on Hemodialysis: A 10-Year Retrospective Study (P6.046)
Conclusion: Peri-procedural and peri-operative strokes are common in patients on Hemodialysis and highest risk is in the period soon after the procedure. Further studies should be directed towards better surgical risk stratification and stroke prevention pre-operatively in these patients.Disclosure: Dr. Sunbulli has nothing to disclose. Renal dysfunction and long-term risk of ischemic and hemorrhagic stroke following coronary artery bypass grafting
Conclusions: Renal dysfunction is associated with increased long-term risk of stroke after primary isolated CABG. The impact of renal dysfunction on risk of stroke appears to be similar for both men and women.

Long-Term Risk of Stroke in Patients With Type 1 and Type 2 Diabetes Following Coronary Artery Bypass Grafting Cardiovascular Surgery
Conclusions The long-term risk for stroke after coronary artery bypass grafting was increased in patients with type 1 and type 2 diabetes, compared to patients with no diabetes.

Influencing Factors for Early Acute Cerebrovascular Accidents in Patients with Stroke History following Off-pump Coronary Artery Bypass Grafting
Conclusions: This study indicated that patients with severe bilateral carotid stenosis, the left ventricular ejection fraction ≤35%, the postoperative acute myocardial infarction, postoperative atrial fibrillation and postoperative hypotension were more likely to suffer from early acute cerebrovascular accidents when they received OPCAB. Application of Enclose® II proximal anastomotic device may decrease the incidence of early acute cerebrovascular accidents during OPCAB.

Cerebrovascular Events After a Primary Percutaneous Coronary Intervention Strategy for Acute ST-Segment- Elevation Myocardial Infarction: Analysis From the HORIZONS-AMI Trial Coronary Interventions
Conclusions— In HORIZONS-AMI, cerebrovascular events within 3 years after ST-segment–elevation myocardial infarction in patients undergoing a primary percutaneous coronary intervention management strategy occurred in 2.0% of patients and were most frequent after coronary artery bypass grafting. Cerebrovascular events were often disabling and were strongly associated with high rates of death, reinfarction, recurrent ischemia, and major bleeding. Clinical Trial Registration— URL: http://www.clinicaltrials.gov. Unique identifier: NCT00433966.

Angioinvasive Aspergillus-associated Stroke in an Immunocompetent Host Following Cardiac Surgery and ECMO
Conclusions: Ischemic infarcts in the setting of CABG or ECMO are often presumed to be thromboembolic from the heart or device, related to underlying hemodynamic instability, or due to a clinically apparent systemic infection such as endocarditis. This report suggests that invasive cerebral aspergillosis should be considered in seemingly immunocompetent patients following CABG or ECMO. The mechanism is unclear, but may be related to systemic inflammatory dysregulation resulting in increased susceptibility to uncommon pathogens.

Carotid artery stenosis at the time of coronary artery bypass grafting is a risk factor but not a cause for peri-operative stroke
Narayan et al. evaluated whether or not carotid artery screening at the time of coronary artery bypass grafting (CABG) influences neurological outcomes [1]. Two of the four strokes in the group with moderate/severe carotid stenosis occurred on the ipsilateral side of the carotid artery lesion, while two were in areas not related to the side of the carotid artery stenosis. In the group with no significant carotid artery stenosis, 18 patients had a stroke of which 16 were ischemic and 2 were hemorrhagic [1].

Rivaroxaban: an oral factor xa inhibitor.
CONCLUSIONS: Based on the findings of the studies reported in this review, rivaroxaban is an effective option for the prevention of VTE after orthopedic surgery, stroke prevention for nonvalvular AF, and treatment of VTE. At this time, rivaroxaban cannot be recommended for secondary risk reduction after ACS because of the increased bleeding risk. PMID: 23328267 [PubMed - in process]

Women With Migraines Are More Prone To This Scary Health Condition
This study provides really good quality evidence that migraine in women is a risk factor for cardiovascular disease," said Dr. Rebecca Burch, an instructor in the department of neurology at Harvard Medical School in Boston, who wrote an editorial accompanying the study, but was not involved in the research. Migraines have a fairly small effect on cardiovascular risk compared with other known risk factors that have a much larger influence on heart disease, such as smoking, high blood pressure or high cholesterol, Burch said. However, because migraines are relatively common, affecting an estimated 1 in 4 American women, this...

Abstract 250: Economic Burden of Mortality and Cardiovascular Events among Patients with Acute Coronary Syndromes in a Commercial Health Plan Poster Session III
Conclusion: Our findings suggest that a modest 10% increase in anticoagulant use among patients with ACS would reduce mortality, MI, ST and related healthcare costs by 4%, 0.7%, and 3%, respectively. Addition of anticoagulation therapy potentially reduces the incidence of ACS-related mortality, MI, ST and associated healthcare costs to a commercial health plan, and benefits from anticoagulation use should be balanced against the risk of bleeding.

Evaluation of brain lesions in patients after coronary artery bypass grafting using MRI with the emphasis on susceptibility-weighted imaging.
CONCLUSIONS: The use of SWI enables one to detect lesions occurring in the brain after CABG, invisible in other sequences. On-pump CABG is associated with a greater risk of clinically silent brain damage compared to off-pump CABG. PMID: 26336470 [PubMed]

Transesophageal Echocardiography Complications in Adult Cardiac Surgery: A Retrospective Cohort Study
Conclusions The overall incidence of TEE complications after cardiac surgery was 1.4%. Advanced age, low body mass index, complexity of procedure, prior stroke, prolonged bypass time, and return to the operating room appear to be significant risk factors for TEE complications.

Abstract 244: Revision of the Consent Form for Coronary Angiography Procedures Session Title: Poster Session II
Conclusion: In a representative assessment of coronary angiography and PCI outcomes, some major complications are higher that those reported in international literature. Significant access site bleeding was low, likely explained by the high prevalence of radial procedures. Future iterations of the patient information sheet and consent form may be improved by providing personalized risks.
