Increase in 18F-Fluoroacetate Uptake in Patients With Chronic Hemodynamic Cerebral Ischemia Brief Reports
Conclusions— In patients with atherosclerotic internal carotid artery or middle cerebral artery disease, 18F-FACE uptake is increased in the noninfarcted cerebral cortex with chronic hemodynamic ischemia characterized by misery perfusion with decreased oxygen metabolism. Increased 18F-FACE uptake may indicate the cortical regions that are at particular risk for ischemic damage.

Detection of Apoptosis with 99mTc rh Annexin-V 128 Imaging in Asymptomatic Patients with Carotid Atherosclerotic Plaque
Conclusions: 99mTc-rhAnnexin V-128 imaging is safe with no observed adverse events or lab result changes. 99mTc-rhAnnexin V-128 imaging of apoptosis is feasible with excellent image quality at one and 2 hours following tracer injection. Significant focal uptake was identified in 1/12 patients and none of the 5 controls. Quantitative analysis of the planar images showed significantly greater uptake of annexin in patients compared to controls for both carotid arteries and at one and two hours.

Superficial temporal artery-middle cerebral artery anastomosis patency correlates with cerebrovascular reserve in adult moyamoya syndrome patients.
CONCLUSION: Our data suggest that improvement in cerebral perfusion and CVR depends on flow in the STA-MCA anastomosis in patients with Moyamoya syndrome. Systematic long-term follow-up of anastomosis flow, brain perfusion and CVR improves quantification of the benefit of STA-MCA anastomosis in terms of disease progression. Moyamoya disease with epileptic nystagmus: A case report
We report a case of moyamoya disease with epileptic nystagmus. A 23-year-old woman presented with a headache and transient hemiparesis on her left side. Magnetic resonance imaging showed no ischemic or hemorrhagic stroke lesions. Multimodal evaluation of the cerebrovascular reserve in Neurofibromatosis type 1 patients with Moyamoya syndrome

Surgical Management of Failed Revascularization in Moyamoya Vasculopathy
Conclusion: Overall, repeat revascularization is needed only in a small percentage of the cases in MMV. A rescue surgery should be considered in those with neurological symptoms and decreased CVRC. Intermediate-flow bypass using a radial artery graft is a reliable technique for patients requiring repeat revascularization. Based on our institutional experience, we propose an algorithm for guiding the decision process in cases of bypass failure.

Which is responsible for aphasia by subcortical lesions? Subcortical lesions or the cortical hypoperfusion?
This study showed that aphasia to be more common in cases with cortical hypoperfusion in the dominant hemisphere than in cases without hypoperfusion.
