Long-Term Outcomes of Stroke or Transient Ischemic Attack after Non-Emergency Percutaneous Coronary Intervention
Elective percutaneous coronary intervention is increasing in volume, which may be due to an increased use of CT coronary angiography as a first-line investigation for the evaluation of chest pain.1,2 Stroke is a debilitating neurological complication with increasing incidence after PCI in recent years, due to an increasing complexity of patients.3 Large observational and randomized controlled studies found that peri-procedural stroke occurred in 0.13-0.4% of unselected patients who underwent PCI.

Simultaneous onset of myocardial infarction and ischemic stroke in a patient with atrial fibrillation: Multiple territory injury revealed on angiography and magnetic resonance
An 84-year-old man with a history of hypertension and paroxysmal atrial fibrillation (AF) who received no anticoagulant drugs experienced acute chest pain and transient loss of consciousness. He was transferred to our emergency room. His initial electrocardiogram showed sinus rhythm with ST-segment elevation in the I, aVL, and V1–V6 leads. His blood pressure was 158/92mmHg and his pulse was regular at 70beats per minute. A chest radiograph showed increased heart size and pulmonary vascular congestion. His troponin T level was elevated (more than 0.1ng/mL). He was diagnosed with ST-elevation myocardial infarction (MI) and...

De novo headache in ischemic stroke patients treated with thrombectomy: a prospective study
ConclusionsAlmost one-third of patients with ischemic stroke who undergo endovascular thrombectomy experience headache in the first 24  hours, occurring more frequently in patients who had a previous history of headaches regardless of the procedure complexity.

A 28-year-old male with spontaneous coronary artery dissection complicated by apical thrombus and acute ischemic stroke: An interesting case
Radiol Case Rep. 2022 Nov 2;18(1):246-249. doi: 10.1016/j.radcr.2022.10.001. eCollection 2023 Jan.ABSTRACTSpontaneous coronary artery dissection is described as the intramural bleeding that separates the layers of an epicardial coronary artery wall, either with or without an intimal tear. Atherosclerosis, iatrogenic damage, or trauma are not linked to this syndrome. Here we present a 28-year-old male with 1 month history stroke but no any chronic disease as well family history of heart disease who presented with 2 days' duration of typical cardiac chest pain. Based on an emergency electrocardiogram that showed biphasic T-w...

A Case of Transient Global Amnesia: A Review and How It May Shed Further Insight into the Neurobiology of Delusions
Conclusion In closing, our patient’s episode of TGA combined with her emotional and perceptual response lends credence to the proposal of a “fear/paranoia” circuit in the genesis of paranoid delusions—a circuit incorporating amygdala, frontal, and parietal cortices. Here, neutral or irrelevant stimuli, thoughts, and percepts come to engender fear and anxiety, while dysfunction in frontoparietal circuitry engenders inappropriate social predictions and maladaptive inferences about the intentions of others.[54] Hippocampus relays information about contextual information based on past experiences and the current situat...

The Biggest Medical Stories You May Have Missed In 2015
SPECIAL FROM Next Avenue By Craig Bowron As we head into the New Year, let’s take a look back and see what lessons we should have learned from medical science in 2015. The New England Journal of Medicine’s publication Journal Watch provides physicians and other health care providers with expert analysis of the most recent medical research. Below is a brief synopsis of what the Journal Watch editors felt were the most important stories in general medicine for the year 2015. While you likely heard about a couple, others probably escaped your radar. Getting Aggressive with Strokes We’re familiar with the id...

Outcome of primary PCI - An Indian tertiary care center experience.
CONCLUSION: Our study has shown that PPCI is feasible with good outcomes in Indian scenario. Even though the recommended door-to-balloon time can be achieved, the total ischemic time remained long. CS in the setting of STEMI was associated with poor outcomes. PMID: 24581092 [PubMed - in process]

Clinical Performance Measures that Matter —Are You Ready?
Conclusion The future will belong to those who can prove value. The use of a clinical performance dashboard and comparisons to national data will help to ensure that when the payers come knocking on our doors looking for "proof" that what we do enhances patient care and improves patient outcome, we’ll have an answer! This is the first in a yearlong series of articles developed by the Academy of International Mobile Healthcare Integration (AIMHI).The AIMHI article series is developed in partnership with JEMS to help educate EMS agencies on the hallmarks and attributes of high-performance/high-value EMS system de...

Patient preferences for coronary artery bypass graft surgery or percutaneous intervention in multivessel coronary artery disease
Conclusion: Patients favor mv‐PCI over CABG to treat mv‐CAD, even if 1‐year risks of death and repeat procedures far exceed risk with CABG. Physicians are more influenced by actual risk and prefer mv‐PCI less than patients despite similarly quoted 1‐year risks. © 2012 Wiley Periodicals, Inc.

Saturated fat link with heart disease questioned
This article is one doctor's opinion based on his own knowledge, research and experience. However, it is fair to say there is an ongoing debate about how far cholesterol is a risk factor for heart disease, especially in people who are otherwise healthy. There is also a similar debate about the use of statins in people who have no evidence of cardiovascular disease. This is alongside ongoing research into the components of LDL and the different types of lipoproteins known to increase risk the most. None of this relevant new evidence is covered by the news reporting.   What should you eat? There is no need to change curren...

Combined carotid endarterectomy and retrograde stenting of the supra-aortic trunk: Does cervical block offers advantages?
Atherosclerosis of the carotid artery is a major source of stroke. In some cases atherosclerosis occurs at several positions within the carotid artery. Carotid endarterectomy (CEA) in combination with retrograde balloon angioplasty and stenting of a brachiocephalic or common carotid artery stenosis has been described as efficacious and save procedure to prevent stroke in these cases. The aim of this study was to analyse the impact of anaesthetic techniques on hemodynamic factors, operation time, duration of clamping and post-operative pain.

P-013 Use of the Solitaire Device for Emergency Revascularization of the Superior Mesenteric Artery
ConclusionsThe techniques and equipment used for acute stroke intervention may be suitable for acute recanalization of peripheral vessels, under the right circumstances. As with many surgical interventions, a multidisciplinary approach may at times yield a novel and effective strategy for a difficult clinical problem.DisclosuresJ. Dalfino: None. A. Paul: None. J. E-027 Matricidal cavernous aneurysms: a multicenter case series
Conclusion Matricidal aneurysms require careful consideration and planning. The restricted anatomy of the cavernous sinus can make successful execution of endovascular interventions difficult. Direct elastic compression of the parent artery does not respond to angioplasty and stenting the way that atherosclerotic stenosis does. Because of this, planning for possible parent vessel sacrifice is important. Abstract E-027 Table 1 Intended Treatments and Outcomes n Intended Treatment Failed Treatment (rate) All Patients 37 10 (27%) Flow Diversion 18 5 (28%) Parent Vessel Sacrifice 11 With Bypass 4 2 (50%) Without Bypass 7 0 ...

New heart op could save 'thousands of lives'
Conclusion This was a well-designed study which showed that over the course of two years, the risk of another non-fatal or fatal heart attack is reduced in heart attack patients who have preventive stents inserted in narrowed arteries at the same time as a procedure to insert stents into the blocked coronary artery that caused their heart attack. However, an important point to note is that the study results do not apply to all patients who have had a heart attack. The study only looked at the group of patients who had a specific type of heart attack – a STEMI – and needed an emergency stent placement operation. ...
