Silent Myocardial Infarction: A Case Report
We present a case of a 59-year-old overweight woman with prediabetes, primary hypertension, and hypercholesterolemia who presented for herpes zoster (HZ) follow-up; she reported having skipped heartbeats and heart rate fluctuations during the review of systems. On further workup, ECG revealed low voltage QRS complexes, flat QRS complexes, flat T waves, and pathological Q waves, suggesting the diagnosis of SMI. Half-Dose Tenecteplase or Primary Percutaneous Coronary Intervention in Older Patients With ST-Segment-Elevation Myocardial Infarction in STREAM-2: A Randomized, Open-Label Trial
CONCLUSIONS: Halving the dose of tenecteplase in a pharmaco-invasive strategy in this early-presenting, older STEMI population was associated with electrocardiographic changes that were at least comparable to those after primary PCI. Similar clinical efficacy and angiographic end points occurred in both treatment groups. The risk of intracranial hemorrhage was higher with half-dose tenecteplase than with primary PCI. If timely PCI is unavailable, this pharmaco-invasive strategy is a reasonable alternative, provided that contraindications to fibrinolysis are observed and excess anticoagulation is avoided.REGISTRATION: URL: ...

Lateral medullary syndrome: uncommon form of brainstem stroke

Mitral Valve Blood Cyst and Multi-Territory Ischaemic Stroke
A 78-year-old male with a history of hypertension and hypercholesterolaemia was admitted to hospital with acute left eye visual loss and diagnosed with retinal artery occlusion. Magnetic resonance imaging (MRI) revealed multiple, bilateral cerebral infarcts involving supra- and infra-tentorial brain consistent with acute ischaemic stroke from a central embolic source. Extended ambulatory electrocardiographic monitoring displayed sinus rhythm with no evidence of atrial flutter or fibrillation. Transthoracic echocardiography (TTE) revealed a cystic structure associated with the mitral valve, with no restriction or regurgitation.

Predictors of 30-day mortality among patients with stroke admitted at a tertiary teaching hospital in Northwestern Tanzania: A prospective cohort study
ConclusionStroke is associated with a high 30-day mortality rate in Northwestern Tanzania. Concerted efforts are warranted in managing patients with stroke, with particular attention to individuals with severe strokes, ECG abnormalities, and swallowing difficulties to reduce early morbidity and mortality.

Predicting Atrial Fibrillation after Ischemic Stroke: Clinical, Genetics, and Electrocardiogram Modelling
