Association of uric acid and carotid artery disease in patients with ischemic stroke
ConclusionOur study demonstrated that higher uric acid level is strongly associated with CAD. Elevated uric acid might be injurious for large cerebral arteries with some probable confounding risk factors. Further prospective large clinical trials will determine whether lowering uric acid level reduces the frequency of CAD and ischemic stroke.

Incidence of stroke and acute renal failure in patients of postoperative atrial fibrillation after myocardial revascularization

Life-threatening heat stroke presenting with ST elevations: a report of consecutive cases during the heat wave in Austria in July 2013.
We report a series of four cases (three exertional, one classic heat stroke) during the heat wave of July 2013 in Austria. All of them presented with a core temperature > 41 °C, central nervous dysfunction, acute respiratory and renal failure, disseminated intravascular coagulation, rhabdomyolysis, and severe electrocardiographic changes, two cases even mimicking ST-elevation myocardial infarction. The patients were cooled to normal temperature with the "Arctic sun" external cooling system within hours. Electrocardiographic changes resolved quickly. All patients primarily recovered from multiple organ dysfunction a...

074-i * off-pump coronary artery bypass reduces early stroke in octogenarians: a meta-analysis of 18,000 patients
Conclusion: Coronary artery bypass in octogenarians can be performed with low early mortality. Number of grafts is lesser in the OPCABG cohort. While stroke rates are higher with conventional surgery, all other adverse events are comparable. Future randomised trials are needed to define the role of off-pump surgery in this high-risk cohort.

Off-Pump Coronary Artery Bypass Reduces Early Stroke in Octogenarians: A Meta-Analysis of 18,000 Patients
Conclusions Coronary artery bypass in octogenarians can be performed safely with low early mortality. Although off-pump operations reduce the risk of early stroke, all other adverse events are comparable in on- and off-pump coronary artery bypass operations. Data regarding late mortality is at present limited; however, both on- and off-pump procedures appear to produce comparable survival.

Development and Validation of Prognostic Indices for Recovery of Physical Functioning Following Stroke: Part 1
Conclusion By using a simple tool, clinicians can forecast the likelihood of recovery to or above the physical grade IV benchmark by the conclusion of rehabilitation services during the acute stroke hospitalization.

Major Bleeding and Hemorrhagic Stroke with Direct Oral Anticoagulants in Patients with Renal Failure: Systematic Review and Meta-Analysis of Randomized Trials.
CONCLUSIONS: As a class, DOACs are associated with reduced risk for hemorrhagic stroke in patients with RF compared to VKA. However, DOACs may differ from each other in their relative risk for major bleeding in patients with eCrCL <50mL/min. PMID: 26836922 [PubMed - as supplied by publisher]

The Epidemiology and Clinical Associations of Stroke in Patients with Acute Myeloid Leukemia: A review of 10,972 admissions from the 2012 National Inpatient Sample
Acute leukemia predisposes patients toward the development of stroke. The latter, while devastating clinically, has been infrequently studied. Our study, using the 2012 National Inpatient Sample, found a 50-fold increase in the risk of stroke as compared to all inpatient admissions with a corresponding 5.5-fold increased risk of mortality. Significant risk factors for the development of stroke included UTI, hypernatremia and acute renal failure.

A Functional Return-to-Play Progression After Exertional Heat Stroke in a High School Football Player.
CONCLUSIONS:   Using a gradual RTP progression and close monitoring, a high school defensive end successfully returned to football practice and games after EHS. This case demonstrates the feasibility of implementing a safe RTP protocol after EHS and may serve as a guide to ATs working in the high school setting. This case also highlights the need for more research in this area. PMID: 29373058 [PubMed - as supplied by publisher]

Prevalence, Predictors, and Outcomes of Prolonged Mechanical Ventilation After Endovascular Stroke Therapy
ConclusionsAmong a nationally representative sample of hospitalizations, nearly one-in-six patients had prolonged mechanical ventilation after EVT. Heart failure and diabetes were significantly associated with prolonged mechanical ventilation following EVT. Prolonged ventilation was associated with significant increase in in-hospital mortality and morbidity.

Severe Hyperkalemia Masquerading as Acute Ischemic Stroke: A Case Report
We report a case of a 63-year-old Caucasian woman, who was admitted to Namazi Hospital, affiliated with Shiraz University of Medical Sciences (Shiraz, Iran) in August 2019. The patient suffered from left-sided weakness and slurred speech for one hour prior to admission. Initially, the patient was treated for acute ischemic stroke, and an intravenous recombinant tissue plasminogen activator (IV-rTPA) was prescribed. However, further investigations showed severe hyperkalemia. Hemiparesis and slurred speech improved significantly with appropriate management of hyperkalemia. To the best of our knowledge, this is the first case...

Factors Associated With Length of Hospitalization in Patients Admitted With Transient Ischemic Attack in United States Clinical Sciences
Conclusions— Approximately 75% of patients admitted with transient ischemic attack stay in the hospital for ≥2 days, with the most important determinants being pre-existing medical comorbidities. Longer duration of hospital stay is associated with 2- to 5-fold greater hospitalization charges.

An Autosomal Dominant Arteriopathy Of The Brain Due To A Novel Mutation In Collagen 4A1 Gene In A Family With Early-Onset Stroke And Leukoencephalopathy (S15.007)
CONCLUSIONS: This codon is highly conserved from mammals to drosophila and is expected to affect the collagen structure by disrupting the repeating G-X-X structure. Different missensechange in the same codon has been reported in spontaneous late-onset intracranial hemorrhage, further supporting a pathologic association of this mutation. This family contributes an additional seven cases to the literature. Our findings also underscore the impact of mutations underlying Mendelian disorders on various sporadic, common cerebrovascular disorders with underlying complex genetics. Stroke volume variation compared with pulse pressure variation and cardiac index changes for prediction of fluid responsiveness in mechanically ventilated patients
Conclusions Baseline stroke volume variation ⩾8.15% predicted fluid responsiveness in mechanically ventilated patients with acute circulatory failure. The study also confirmed the ability of pulse pressure variation to predict fluid responsiveness.

Is left atrial appendage closure useful for stroke prevention in atrial fibrillation patients with renal failure?
With great interest we read the article by Kefer et al. about the impact of chronic kidney disease on left atrial appendage (LAA) occlusion with the ACP device for stroke prevention in patients with atrial fibrillation (AF) [1]. We agree with the authors that stroke prevention in AF patients with chronic kidney disease is a problem due to increased bleeding complications during oral anticoagulant therapy with vitamin K antagonists. Furthermore, the use of non-vitamin K-antagonist oral anticoagulants (NOACs) is dangerous or contraindicated in these patients.
