E-228 Single center experience using 3mm trevo stent retrievers in medium vessel occlusion thrombectomy
ConclusionUse of 3mm Trevo NXT stent retriever in medium vessel occlusion for first line and rescue thrombectomy in our single center experience demonstrated an adequate safety and efficacy profile, thus endorsing its use in mechanical thrombectomy applications beyond large vessel occlusion cases. Future multicenter studies will build upon this initial experience to further characterize the utility of novel stent retrievers in medium vessel occlusions.Disclosures C. Zhao: None. M. E-229 Mechanical thrombectomy in low alberta stroke program early CT score (ASPECTS) in hyperacute stroke - a systematic review and meta-analysis
ConclusionMT in hyperacute stroke patients with low ASPECTS (≤5) may result in ambulatory clinical outcomes with acceptable hemorrhage risk. Although a high proportion of patients achieved successful recanalization, this benefit did not fully translate to better clinical outcomes.Abstract E-229 Figure 1Disclosures A. Baig: None. M. E-231 Long-term safety and efficacy of the pipeline embolization device for treatment of small vs. large aneurysms: a systematic review and meta-analysis
ConclusionThe long-term occlusion rate after PED treatment is higher in large vs. small aneurysms. Symptomatic thromboembolic rates with stroke are also higher in large vs. small aneurysms. The difference in the rates of aneurysm rupture and intracranial hemorrhage was insignificant. Although the PED seems a safe and effective treatment for small and large aneurysms, further studies are required to clarify how occlusion rate and morbidity are affected by aneurysm size.Disclosures A. E-236 Automated pre- and post-operative volumes estimates risk of retreatment in chronic subdural hematoma: a retrospective, multi-center study
Conclusions/RelevanceLarger pre- and post-operative cSDH volumes increase the risk of cSDH retreatment. Volume thresholds may allow identification of patients at high risk of cSDH retreatment who would benefit from adjunct treatments. Machine learning algorithm can quickly provide accurate estimates of pre and post operative volumes.Disclosures J. E-245 Association of pre-treatment cerebral microbleeds with hemorrhagic transformation following successful endovascular recanalization in patients with acute ischemic stroke arising from large artery occlusion
ConclusionsThese findings suggest that the presence of CMBs is associated with HT and poor functional outcome following EVT and had no effect on mortality in patients with successful endovascular recanalization. However, CMBs do not show such a high impact on HT as other literature in our study. Therefore, it seems appropriate to consider CMB status in individual risk stratification of poor clinical outcome following recanalization by EVT with AIS. Future large and well-designed prospective studies are required to better characterize the underlying factors behind CMB-associated clinical deterioration.Abstract E-245 Figure ...

E-247 The universal effect of the 'golden hour on stroke thrombectomy outcomes
ConclusionsIn this work we demonstrate the universality of the ‘Golden Hour’ effect, in which procedures lasting more than 1 hour are associated with worse clinical outcomes and higher rates of symptomatic hemorrhage. This study will assist clinicians in determining when to be more aggressive in their approach when aiming for TICI 3 reperfusion in different subpopulations of patients.Disclosures M. Ash: None. L. E-257 Flow diversion of the ophthalmic aneurysms, outcomes in relation to the ophthalmic artery origin
ConclusionFlow diversion is safe and effective for carotid ophthalmic aneurysms treatment. Adjunct coil embolization and ophthalmic artery origin in relation to the aneurysm sac are independent factors that can influence treatment result, and should be considered when planning treatment.Disclosures M. E-260 Bilateral cerebral arteriovenous shunting through pial and perforating vessels with multiple strokes and intraparenchymal hemorrhages in a patient with hepatopulmonary syndrome
This study presents a unique case of a 54-year-old male patient with a history of stroke, liver cirrhosis, portal vein thrombosis, hypertension, diabetes, and bladder cancer, who experienced multiple episodes of intracranial hemorrhages, stroke and worsening confusion over the past five years. The patient‘s clinical presentation raised suspicion for an underlying occult vasculopathy. Digital subtraction angiography (DSA) revealed multiple curvilinear, irregular, bilateral pial vessels with early high-flow arteriovenous shunting, consistent with vascular shunting that is associated with liver cirrhosis. The patient&ls...

E-261 Variability of response on prophylactic prasugrel for endovascular treatment of intracranial aneurysms: clinical implications
ConclusionIn the neurointerventional field, high BMI and prior use of antithrombotic agents were related to HTPR, and low BMI was associated with LTPR on prophylactic prasugrel. High hematocrit levels decreased the risk of HTPR. When preparing endovascular treatment for intracranial aneurysms, attention to patients with these clinical features is required to address the possibility of ischemic or bleeding complications.Abstract E-261 Table 1Risk factor analysis of high on-treatment platelet reactivity (HTPR) Variables Univariate Analysis Multivariate Analysis OR (95% CI) P OR (95% CI) P Age 1.02 (0.97-1.06) 0.53 Female o...

E-262 Pipeline embolization device for the treatment of unruptured intracranial saccular aneurysms: a systematic review and meta-analysis of long-term outcomes

Time-to-treatment with endovascular thrombectomy in patients with large core ischemic stroke: the 'late window paradox
The results of the recently published randomized clinical trials of endovascular thrombectomy (EVT) for large vessel occlusion (LVO) in acute ischemic stroke (AIS) patients with a large ischemic core: ANGEL-ASPECT,1 SELECT-2,2 and the RESCUE-Japan LIMIT trials,3 are promising and are expected to lead to more AIS-LVO patients undergoing EVT. Prior studies have demonstrated that shorter time-to-treatment has been found to be directly associated with better functional outcomes in AIS-LVO patients undergoing EVT.4 In this short commentary, we have pooled the available data from the abovementioned trials to conduct a subgroup a...
