O-005 Submaximal angioplasty alone vs. stenting plus angioplasty for symptomatic intracranial atherosclerotic disease - a propensity score matched comparative study with long term follow-up
ConclusionSubmaximal angioplasty alone results in better long-term clinical outcomes in terms of stroke/TIA when compared with stenting and angioplasty for symptomatic intracranial atherosclerotic disease, and represents a potentially promising intervention.Abstract O-005 Table 1Procedural outcomes and patient follow-up dichotomized by submaximal angioplasv vs. stenting and angioplasty after propensity score matchingDisclosures A. O-007 Bridging thrombolysis yields diminished benefit among elderly stroke patients treated with endovascular thrombectomy
ConclusionsBridging thrombolysis may be associated with better outcomes in a real-world setting; however, this association is diminished among elderly patients due to higher rates of hemorrhagic complications. Meta-analyses of trial data and future prospective studies are needed to fully elucidate the benefits and risks of bridging thrombolysis in the elderly population.Abstract O-007 Figure 1Disclosures H. O-008 Endovascular thrombectomy outcomes stratified by hospital thrombectomy volume: a national inpatient sample study
ConclusionsPatients receiving EVT for AIS at HV centers were more likely to be discharged to home without assistance but demonstrated lower rates of in-hospital mortality when compared to IV and LV centers. Our findings support HV EVT performing centers render more favorable outcomes in this NIS analysis.Disclosures A. Brake: None. L. Fry: None. C. Heskett: None. G. LeBeau: None. C. Lei: None. A. O-010 Tigertriever in the treatment of acute ischemic stroke in patients with underlying intracranial atherosclerotic disease

O-011 Tenzing 7 delivery catheter system with or without a leading microwire for acute stroke aspiration thrombectomy: a multicenter experience
ConclusionsRates of successful catheter delivery using the Tenzing 7 device amongst single pass aspiration thrombectomies were similar with and without the use of microwires. The majority of cases did not require a microwire. T7 thrombectomies without microwires were functionally noninferior to procedures using a microwire; the former is more cost-effective as fewer devices are used. From a technique standpoint, initial pass with T7 can be performed without use of microwire, reserving microwire use for refractory cases or known difficult-to-navigate vasculature.Disclosures M. O-012 Relationship between clot composition and procedural results for ischemic stroke aspiration thrombectomy: subset analysis from the INSIGHT registry
ConclusionsClot removed during neuroendovascular procedures can differ in consistency and removability, leading to an increased focus on thrombus composition as a crucial variable for improving outcomes of ischemic stroke. In this study, good final revascularization (mTICI 2b-3 at the final angiogram) occurred after aspiration thrombectomy regardless of clot composition. Ongoing analysis of collected specimens is expected to provide valuable information regarding thrombus biology and etiology and to aid in diagnosis, prognosis, and drug development.Abstract O-012 Table 1Clot composition vs procedural results for ischemic s...

O-014 Mechanical thrombectomy for the treatment of primary and secondary anterior cerebral artery occlusions--insights from STAR
ConclusionClinical and procedural outcomes for MT-treated ACA occlusions can vary based on etiology (ie. primary, secondary). While rate of successful recanalization and 90-day mortality were similar, secondary ACA occlusions were associated with higher rates of morbidity compared to primary ACA occlusions. Our findings demonstrate the need for an established criteria to determine ideal patient and ACA stroke characteristics amendable to MT treatment. Randomized control trials are necessary to further examine differences in outcomes based on etiology of ACA occlusion.Disclosures N. Dabhi: None. J. Kumar: None. N. Ironside:...

O-015 The safety and efficacy of cangrelor use in endovascular thrombectomy compared with glycoprotein IIb/IIIa inhibitors

O-018 Utilizing DSC MR perfusion images for evaluation of hemorrhagic transformation after mechanical thrombectomy with Heidelberg grading and comparison to SWI, GRE, and CT
ConclusionAmong the 3 acquisitions, HT was identified more often on SWI/GRE than on CT or MR perfusion. For HT more likely to be clinically significant (Heidelberg grade 2 or higher), both SWI/GRE and DSC-MR perfusion performed similarly to CT. Thus, when obtaining DSC MR perfusion images for ischemic stroke imaging, SWI/GRE may not be necessary for evaluating HT.Abstract O-018 Figure 1Disclosures C. Reynolds: None. B. Renshaw: None. S. O-026 Incremental value of arterial spin labeling (ASL) over conventional MRI sequences in the detection of intracranial arteriovenous malformations in patients with hereditary hemorrhagic telangiectasia (HHT)
ConclusionsOur findings underscore the incremental value of ASL in addition to conventional MRI sequences as a screening tool in the detection of brain AVMs in patients with HHT. ASL was more sensitive than conventional contrast-enhanced MRI alone, potentially improving diagnostic accuracy in this patient population with many small AVMs. These findings further highlight the need for further research to explore the benefits of integrating ASL sequences into the initial evaluation and routine follow-up of patients with sporadic and HHT-related brain AVMs.Abstract O-026 Figure 1Disclosures W. O-027 Contrast density and volume on post-thrombectomy dual-energy head computed tomography predict delayed hemorrhagic transformation
ConclusionMaximum contrast density predicted the occurrence of delayed petechial hemorrhage, while contrast volume predicted the occurrence and volume of delayed parenchymal hematoma. O-035 Endovascular treatment of cerebral venous thrombosis involving the deep venous system
ConclusionsAlthough deep venous involvement of CVT is uncommon, these patients are at risk of quickly becoming critically ill with an elevated rate of death or dependency. Although these patients may develop stroke, hemorrhage, or neurologic decline prompting consideration of endovascular therapy, the deep cerebral venous system is not easily accessible with catheters due to small lumens and risk of perforation. It has been proposed that deep cerebral veins may have a higher chance of recanalization if venous thrombus burden is reduced in other parts of the dural venous sinuses. Current literature has not fully explored th...
