Sex-related differences in dynamic right ventricular-pulmonary vascular coupling in heart failure with preserved ejection fraction.
CONCLUSION: Male HFpEF patients were more compromised with regards to dynamic RV-PA uncoupling and reduced peak VO2 compared to females. This was driven by both RV contractile impairment and afterload mismatch. In contrast, female HFpEF patients had preserved RV-PA coupling during exercise and better peak exercise VO2 compared to male HFpEF. PMID: 33388286 [PubMed - as supplied by publisher]

The Relationship Between Asthma and Cardiovascular Disease: An Examination of the Framingham Offspring Study.
Abstract BACKGROUND: Although asthma has been suggested as a risk factor for cardiovascular disease (CVD), robust longitudinal evidence of this relationship is limited. RESEARCH QUESTION: Using Framingham Offspring Cohort data, we sought to longitudinally examine the association between asthma and lifetime risk of CVD while controlling for cardiovascular risk factors included in the Framingham Risk Score. STUDY DESIGN AND METHODS: We analyzed data from a prospective population-based cohort of 3,612 individuals, ages 17 to 77 years, who participated in Framingham Offspring Study examinations from 1979 to ...

Central sleep apnea predicts pulmonary complications after cardiac surgery.
The objectives of the present study were to determine the incidence of the composite endpoint postoperative MPCs and to identify predictors for postoperative MPCs in patients without SDB, with obstructive sleep apnea (OSA) and with central sleep apnea (CSA) undergoing cardiac surgery. STUDY DESIGN: and methods: In this sub-analysis of the ongoing prospective observational study CONSIDER AF, preoperative risk factors for postoperative MPCs were examined in 250 patients undergoing cardiac surgery. Postoperative MPCs, including respiratory failure, acute respiratory distress syndrome, pneumonia, or pulmonary embolism, we...

Aspiration risk factors, microbiology and empiric antibiotics for patients hospitalized with community-acquired pneumonia.
Abstract BACKGROUND: Aspiration community-acquired pneumonia (ACAP) and community-acquired pneumonia (CAP) in patients with aspiration risk factors (AspRFs) are infections associated with anaerobes, but limited evidence suggests their pathogenic role. RESEARCH QUESTION: What are the aspiration risk factors, microbiology patterns, and empiric anti-anaerobic use in patients hospitalized with CAP? STUDY DESIGN AND METHODS: This is a secondary analysis of GLIMP, an international, multicenter, point-prevalence study of adults hospitalized with CAP. Patients were stratified into three groups: 1) ACAP, 2) CAP/A...

Self-reported snoring patterns predict stroke events in high-risk patients with obstructive sleep apnea: post-hoc analyses of the SAVE study.
Abstract BACKGROUND: The relation of snoring to risks of stroke and other major cardiovascular (CV) events is uncertain. RESEARCH QUESTION: We aimed to determine associations of snoring patterns and major CV events in relation to the obstructive sleep apnea (OSA), among participants of the international Sleep Apnea cardiovascular Endpoints (SAVE) trial. STUDY DESIGN: and Methods: Post-hoc analyses of the SAVE trial, which involved 2687 patients with co-existing moderate-severe OSA and established coronary or cerebral CV disease, who were randomly allocated to continuous positive airway pressure (CPAP) tr...

Fluid Response Evaluation in Sepsis Hypotension and Shock: A Randomized Clinical Trial.
Abstract BACKGROUND: Fluid and vasopressor management in septic shock remains controversial. In this randomized controlled trial, we evaluated the efficacy of dynamic measures (stroke volume change during passive leg raise) to guide resuscitation and improve patient outcome. RESEARCH QUESTION: Will resuscitation guided by dynamic assessments of fluid responsiveness in patients with septic shock improve patient outcomes? STUDY DESIGN: and Methods: Prospective, multicenter, randomized clinical trial at 13 hospitals in the United States and United Kingdom. Patients presented to Emergency Rooms with sepsis a...

Integrating the STOP-BANG score and clinical data to predict cardiovascular events after infarction: A machine learning study.
Abstract BACKGROUND: Obstructive sleep apnea (OSA) conveys worse clinical outcomes in coronary artery disease patients. The STOP-BANG score is a simple tool that evaluates the risk of OSA and can be added to the large number of clinical variables and scores obtained during the management of myocardial infarction (MI) patients. Currently, machine learning (ML) is able to select and integrate numerous variables to optimize prediction tasks. RESEARCH QUESTION: Can the integration of STOP-BANG score with clinical data and scores through ML better identify patients who suffered an in-hospital cardiovascular event ...

Echocardiography-derived stroke volume index is associated with adverse in-hospital outcomes in intermediate-risk acute pulmonary embolism: a retrospective cohort study.
Abstract BACKGROUND: There remains uncertainty in the optimal prognostication and management of patients with intermediate-risk PE. Transthoracic echocardiography can identify right ventricular (RV) dysfunction to recognise intermediate-high risk patients. RESEARCH QUESTION: The aim of this study was to test whether echocardiography-derived stroke volume index (SVI) is associated with death or cardiopulmonary decompensation in intermediate-risk patients with pulmonary embolism (PE). STUDY DESIGN AND METHODS: We retrospectively evaluated echocardiographic-derived variables including SVI in normotensive pa...

Isolated pulmonary embolism is associated with a high risk of arterial thrombotic disease - Results from the VTEval Study.
Abstract BACKGROUND: Isolated PE is associated with a higher burden of atherosclerotic disease than other manifestations of venous thromboembolism (VTE). RESEARCH QUESTION: We hypothesized that the presence of isolated PE may signal a chronically elevated risk of arterial thrombotic disease. STUDY DESIGN AND METHODS: Data from the VTEval Study, a prospective cohort study enrolling individuals with clinical suspicion and imaging-based diagnosis or exclusion of VTE, were analyzed. Patients with PE received whole-leg ultrasonography to assess presence of deep vein thrombosis (DVT). Regularized logistic regr...

Mortality of cardiovascular events in COPD patients with preceding hospitalized acute exacerbation.
Abstract BACKGROUND: Acute exacerbation (AE) of chronic obstructive pulmonary disease (COPD) may be accompanied by the deterioration of cardiovascular comorbidities, as evidenced by the increased incidence of acute cardiovascular events. RESEARCH QUESTION: Whether preceding AE might be associated with mortality of cardiovascular events remains unknown. STUDY DESIGN AND METHODS: Using a health insurance research database in Taiwan, we identified patients with COPD who experienced first-time acute myocardial infarction (AMI; n=26,442), ischemic stroke (n=54,959) and intracranial hemorrhage (ICH; n=14,893) ...
