Prevalence, incidence and lifetime risk of atrial fibrillation in China: New insights into the global burden of atrial fibrillation.
CONCLUSION AF burden, as well as the risk of AF-related stroke, has increased significantly over past 11 years in the southwest of China. The public health burden of AF and its complications ere greatest in the very elderly, with major implications for healthcare systems given the global burden of this common arrhythmia. Patent foramen ovale and stroke in intermediate-risk pulmonary embolism.
CONCLUSIONS: PFO and related ischemic strokes are frequent in intermediate-risk pulmonary embolism. TEE is much more efficient than TTE for PFO diagnostic. Considering the high risk of intra-cranial bleeding with thrombolysis in PE, which in part may be due to haemorrhagic transformation of sub-clinical strokes, screening PFO with TEE should be considered in intermediate risk PE when a thrombolytic treatment is discussed. Stroke/thromboembolism and intracranial hemorrhage in a 'real world' atrial fibrillation population: The CAFBO STUDY (Complications of Atrial Fibrillation in the BOlogna area).
CONCLUSION IEs occurred mainly in elderly patients who received ASA or no treatment. Half of anticoagulated patients with IEs had subtherapeutic INRs. Therapeutic approaches to elderly subjects with AF require an effective anticoagulant treatment strategy. PMID: 24810397 [PubMed - as supplied by publisher]

Obstructive sleep apnea is common and independently associated with hypertension and increased arterial stiffness in consecutive perimenopausal women.
CONCLUSIONS: OSA is common, underdiagnosed and independently associated with high blood pressure and increased arterial stiffness in the perimenopause. PMID: 24743835 [PubMed - as supplied by publisher]

Long term outcomes following development of new-onset atrial fibrillation during sepsis.
CONCLUSIONS: Most sepsis survivors with new-onset AF during sepsis have AF occur after discharge from the sepsis hospitalization, and have increased long-term risks for heart failure, ischemic stroke and death. Our findings may have implications for post-hospitalization surveillance of patients with new-onset AF during a sepsis hospitalization. PMID: 24723004 [PubMed - as supplied by publisher]

Relationship of the SAME-TT2R2score to poor quality anticoagulation, stroke, clinically relevant bleeding and mortality in patients with atrial fibrillation.
CONCLUSION We demonstrate that the SAME-TT2R2 score was predictive for an increasing risk of stroke/TE, severe bleeding, major BARC bleeding and death, reflecting poor anticoagulation control (and labile INRs) on VKA amongst our patients with AF. PMID: 24722973 [PubMed - as supplied by publisher]

Clinical applications of targeted temperature management.
Abstract Targeted temperature management (TTM) has been investigated experimentally and used clinically for over 100 years. The initial rationale for the clinical application of TTM, historically referred to as therapeutic hypothermia, was to decrease the metabolic rate, allowing the injured brain time to heal. Subsequent research demonstrated the temperature dependence of diverse cellular mechanisms including endothelial dysfunction, production of reactive oxygen species, and apoptosis. Consequently, modern use of TTM centers on neuroprotection following focal or global neurologic injury. Despite a solid basic sc...

Advanced echocardiography for the critical care physician: part 1.
Abstract This is the first of a two-part series that reviews advanced critical care echocardiography (CCE) techniques designed for critical care physicians. In this section, we review training in basic and advanced CCE. This is followed by a review of Doppler principles, including pulsed wave, continuous wave, and color flow Doppler. Included are Doppler measurement techniques that are useful for assessing the patient with cardiopulmonary failure and the common pitfalls of Doppler. This section ends with a review of the quantitative and semiquantitative measurements of stroke volume, as well as problems with measu...

Leveraging a critical care database: SSRI use prior to ICU admission is associated with increased hospital mortality.
ConclusionsWe found significant increases in hospital stay mortality among those ICU patients on SSRI/SNRIs prior to admission as compared to controls. Mortality was higher in patients receiving those SSRI/SNRI agents that produce greater degrees of serotonin reuptake inhibition. The study serves to demonstrate the potential for the future application of advanced data examination techniques upon detailed (and growing) clinical databases being made available by the digitization of medicine. Prognostic relevance of pulmonary arterial compliance in patients with chronic heart failure.
CONCLUSIONS Pulmonary arterial compliance is a strong prognostic indicator in CHF patients with sLVD; most importantly, its prognostic role is retained in patients with normal pulmonary vascular resistance. PMID: 24356904 [PubMed - as supplied by publisher]

A prospective study of estimated glomerular filtration rate and outcomes in patients with atrial fibrillation: The Loire Valley Atrial Fibrillation Project.
CONCLUSION Incidence rates of IS/TE, mortality and bleeding increased with reducing eGFR, across the whole range of renal function. OAC use was associated with a lower incidence of IS/TE and mortality at 1 year, compared with non-anticoagulated individuals in all categories of renal function as measured by eGFR. The NCB balancing IS against serious bleeding was positive, in favour of OAC use amongst patients with renal impairment. PMID: 24356875 [PubMed - as supplied by publisher]

The impact of smoking on thromboembolism and mortality in patients with incident atrial fibrillation: insights from the Danish Diet, Cancer and Health study.
CONCLUSIONS Smoking is associated with a higher risk of thromboembolism or death in patients with AF even after adjusting for well-recognised risk factors used in stroke risk stratification schemes. The associations may be modified by sex, as the associations were strongest among women. PMID: 24091709 [PubMed - as supplied by publisher]

Development of a novel composite stroke and bleeding risk score in patients with atrial fibrillation: The AMADEUS study.
CONCLUSION We have developed and validated 2 novel composite scores for stroke/thromboembolism/ bleeding that offer good discriminatory and predictive performance. However, these composite risk scores did not perform better than the easier and more practical 'traditional' stroke and bleeding risk scores that are currently in use, which allow greater practically and more personalised balancing of risks.
