Lifetime Risks, Projected Numbers and Adverse Outcomes in Asian Patients with Atrial Fibrillation: A Report from the Taiwan Nationwide AF Cohort Study.
CONCLUSIONS: The burden of AF amongst Asian patients is increasing, with a lifetime risk of AF being appropriately 1 in 7. Optimized management of any associated comorbidities should be part of the holistic management approach for AF. PMID: 29017957 [PubMed - as supplied by publisher]

Clinical Impact of Asymptomatic Presentation Status in Patients with Paroxysmal and Sustained Atrial Fibrillation: The Fushimi AF Registry.
CONCLUSIONS: Asymptomatic clinical status is associated with older age, males, more co-morbidities with higher stroke risk profile, and a higher incidence of all-cause death in patients with PAF, whereas these characteristics and outcomes were not seen in the SAF group. PMID: 28823813 [PubMed - as supplied by publisher]

Five Year Trends of Critical Care Practice and Outcomes.
CONCLUSIONS: Analyses of patients, practices, and outcomes from a large geographically dispersed sample of adult ICUs revealed trends of increasing age and acuity, higher rates of adherence to best practice, use of non-invasive mechanical ventilation and decreased use of antimicrobials, transfusions, and duration of renal replacement therapies. Acuity adjusted LOS and in hospital mortality decreased. PMID: 28800866 [PubMed - as supplied by publisher]

Hyperbaric oxygen therapy is associated with lower short- and long-term mortality in patients with carbon monoxide poisoning.
CONCLUSIONS: HBOT is associated with a lower mortality rate in patients with COP, especially in those who were younger than 20 years and those with acute respiratory failure. The results provide important references for decision making in the treatment of COP. PMID: 28427969 [PubMed - as supplied by publisher]

The Global Burden of Atrial Fibrillation and Stroke: A Systematic Review of the Clinical Epidemiology of Atrial Fibrillation in Asia.
CONCLUSIONS: AF incidence and prevalence has increased in recent years, though great variability still exists in Asian countries. Variability in annual IS risk in AF patients was apparent between hospital- and community-based studies. However, the rate of warfarin use was less than 50% in hospital studies from Asian countries. PMID: 28427968 [PubMed - as supplied by publisher]

Restrictive Spirometry Pattern is Associated with Increased Arterial Stiffness in Men and Women.
CONCLUSIONS: Both restrictive spirometry pattern and reduced FVC were associated with a higher risk of arterial stiffness, not only in males but also in females. Clinically, assessment of arterial stiffness might be considered in individuals with restrictive spirometry pattern. PMID: 28411113 [PubMed - as supplied by publisher]

Idiopathic and Systemic Sclerosis associated Pulmonary Arterial Hypertension: A Comparison of Demographic, Haemodynamic and Magnetic Resonance Imaging Characteristics and Outcomes.
CONCLUSION: The reasons for poorer outcomes in SSc-PAH are likely to be multifactorial including, but not limited to, older age and reduced gas transfer. PMID: 28223154 [PubMed - as supplied by publisher]

Allogeneic human mesenchymal stem cells in patients with idiopathic pulmonary fibrosis via intravenous delivery (AETHER): a phase I, safety, clinical trial.
CONCLUSIONS: Data from this trial support the safety of a single infusion of hMSC in patients with mild-moderate IPF. PMID: 27890713 [PubMed - as supplied by publisher]

Discriminating Circulatory Problems from Deconditioning - Echocardiogram and Cardio-Pulmonary Exercise Test Analysis.
CONCLUSION: The best single gas exchange parameter to discriminate between circulatory problems to de-conditioning is VE/VCO2 slope to peak VO2 ratio. PMID: 27742182 [PubMed - as supplied by publisher]

Long-acting bronchodilator initiation in COPD and the risk of adverse cardio-pulmonary events: A population-based comparative safety study.
CONCLUSION: COPD treatment initiation with tiotropium compared with LABA does not increase cardiovascular risk in the first year of treatment. The risk of pneumonia is higher with LABA, a likely effect of the inhaled corticosteroids present in many LABA inhalers used in real world clinical practice. PMID: 27554300 [PubMed - as supplied by publisher]

Acute Effects of Nasal CPAP in Patients with Hypertrophic Cardiomyopathy.
CONCLUSIONS: The acute application of CPAP is apparently safe in patients with HCM. However, obstructive-HCM patients should be carefully monitored when initiated on CPAP due to its potential effects on pulmonary pressure. PMID: 27167210 [PubMed - as supplied by publisher]

Variation in Warfarin Use at Hospital Discharge After Isolated Bioprosthetic Mitral Valve Replacement - An Analysis of the Society of Thoracic Surgeons Adult Cardiac Surgery Database.
CONCLUSIONS: Although patient characteristics and post-operative events may be associated with the prescription of warfarin following BMVR, substantial surgeon and hospital variability remains. This variability largely ignores the established practice guidelines, and warrants further study to define the optimal anticoagulation strategy in BMVR patients. PMID: 27132702 [PubMed - as supplied by publisher]

Major Bleeding and Hemorrhagic Stroke with Direct Oral Anticoagulants in Patients with Renal Failure: Systematic Review and Meta-Analysis of Randomized Trials.
CONCLUSIONS: As a class, DOACs are associated with reduced risk for hemorrhagic stroke in patients with RF compared to VKA. However, DOACs may differ from each other in their relative risk for major bleeding in patients with eCrCL <50mL/min. PMID: 26836922 [PubMed - as supplied by publisher]

Longterm course of depression trajectories in patients with COPD: A three year follow-up analysis of the Evaluation of COPD Longitudinally to Identify Predictive Surrogate Endpoints (ECLIPSE) cohort.
CONCLUSION: About one in four of COPD patients had persistent depressive symptoms over 3 years. Clinicians should be aware of the characteristics of persistent and new onset depressive symptoms, which are associated with risk of exacerbations and loss of performance on the 6 minute walk. Interventions that amileroate the course of depression are needed. PMID: 26836938 [PubMed - as supplied by publisher]

Conclusion: Among AF patients with one single additional stroke risk factor (CHA2DS2-VASc score = 1 in males, 2 in females), OAC use was associated with an improved prognosis for stroke/systemic thromboembolism/death. PMID: 26425935 [PubMed - as supplied by publisher]
