Racial differences and disability after stroke
In their article "Racial differences in disability after stroke: Results from a nationwide study," Burke and colleagues1 tried to better understand how race might affect recovery after a stroke. The problem of stroke is large: about 7,000,000 people in the United States are survivors of stroke. That number is getting bigger. Stroke is a problem that mainly affects older individuals. As the US population ages, there will be more people at risk for stroke. In addition, modern medical care has reduced the risk of dying from a stroke, meaning that there will be more survivors of this serious medical illness.

Subclinical cardiac dysfunction increases the risk of stroke and dementia: The Rotterdam Study
Conclusions: In elderly free of clinical cardiac disease, worse diastolic function is associated with clinical stroke, dementia, and silent infarcts on MRI, whereas worse systolic function is related only to clinical stroke. These findings can form the basis for future research on the utility of cardiac function as potential intervention target for prevention of neurologic diseases.

Risk of ischemic stroke during periods of warfarin discontinuation for surgical procedures: A longitudinal study of 4060 patients with atrial fibrillation (P1.058)
CONCLUSIONS: The risk associated with discontinuation of warfarin for procedures must be recognized and considered in the risk benefit analysis of any procedure.Disclosure: Dr. Jahangir has nothing to disclose. Mechanisms and Outcomes of Ischemic Stroke in Pregnancy and Postpartum: A Retrospective Cross-Sectional Study (P3.070)
CONCLUSIONS: Women with pregnancy-associated ischemic strokes were younger and had fewer vascular risk factors when compared with other young women with ischemic strokes. Outcomes were similar in the two groups, with a substantial proportion of poor outcomes in both groups. Our results are limited by the small sample size and retrospective nature of the study. Larger studies are needed to better characterize the pathophysiology of pregnancy-associated stroke.Disclosure: Dr. Miller has nothing to disclose. Dr. Yaghi has nothing to disclose. Quality of life in patients with TIA and minor ischemic stroke
Conclusions: Impairment in HRQOL is common at 3 months after MIS and TIA. Predictors of impaired HRQOL include age, index stroke severity, and recurrent stroke. Future studies should include HRQOL measures in outcome assessment, as these may be more sensitive to mild deficits than traditional disability scales.

Endovascular vs medical management of acute ischemic stroke
Conclusions: This meta-analysis provides strong evidence that endovascular intervention combined with medical management, including IV tissue plasminogen activator for eligible patients, improves the outcomes of appropriately selected patients with acute ischemic stroke in the setting of LVO.

IV thrombolysis in very severe and severe ischemic stroke: Results from the SITS-ISTR Registry
Conclusions: Our data show no excess risk of cerebral hemorrhage in patients with NIHSS score >25 compared to score 15–25, suggesting that the European contraindication to IV tissue plasminogen activator treatment at NIHSS levels >25 may be unwarranted. Increased mortality and lower rates of functional independence in patients with NIHSS score >25 are explained by higher stroke severity, impaired consciousness on presentation due to posterior circulation ischemia, and longer treatment delays.

Oxidative phosphorylation and lacunar stroke: Genome-wide enrichment analysis of common variants
Conclusions: Our results implicate the OXPHOS pathway in the pathogenesis of lacunar stroke, and show the association is specific to patients with the MLI/LA subtype. They show that MRI-based subtyping of lacunar stroke can provide insights into disease pathophysiology, and imply that different radiologic subtypes of lacunar stroke subtypes have distinct underlying pathophysiologic processes.

Incidence, short-term outcome, and spatial distribution of stroke patients in Ludhiana, India
Conclusions: The incidence rates are similar to other studies from India. Stroke patient survival is poor in public hospitals. The finding of spatial analysis is of public health significance for stroke prevention and strengthening of stroke services.
