Intravenous Alteplase for Acute Ischemic Stroke in Taiwan: Can We Expand the National Health Insurance's Reimbursement Criteria?
In conclusion, the non-reimbursement group in Su, et al(3). is a mixture of stroke patients with very heterogeneous characteristics. With the advances of imaging selection modalities and new hyperacute treatment options nowadays, they should no longer be considered as an identical group of patients. We strokologists, as an emerging subspecialists, should devote more in dedicated and individualized selection of stroke patients for appropriate early recanalization therapy to improve their outcomes. Memphis Mobile Stroke Unit Saves Lives
A year after a mobile stroke unit hit the streets of Memphis, officials say it is saving lives by cutting the time it takes to treat stroke patients. The University of Tennessee Health Science Center began using the 14-ton ambulance last year as part of a three-year controlled study. The unit has a CT scanner that can X-ray images of the brain, and is equipped to provide early fluids to stroke patients and dye blood vessels to determine the type of stroke the patient has had. "The emergency room on average takes 40 to 50 minutes to do the things we do in 13 to 14 minutes," said Dr.Andrei Alexandrov, UTHSC neurolo...

Effect of Acute Stroke Care Regionalization on Intravenous Alteplase Use in Two Urban Counties.
Conclusions: Our findings suggest that greater post-regionalization improvements in San Mateo County contributed to significantly better county-level thrombolysis use than Santa Clara County. PMID: 31599705 [PubMed - as supplied by publisher]

Current State of Stroke Care in the Philippines
Stroke remains the leading cause of disability and death in the Philippines. Evaluating the current state of stroke care, the needed resources, and the gaps in health policies and programs is crucial to decrease stroke-related mortality and morbidity effectively. This paper aims to characterize the Philippines' stroke system of care and network using the World Health Organization health system building blocks framework. To integrate existing national laws and policies governing stroke and its risk factors dispersed across many general policies, the Philippine Department of Health (DOH) institutionalized a national policy f...

Tissue‐factor‐bearing microparticles (MPs‐TF) in patients with acute ischaemic stroke: the influence of stroke treatment on MPs‐TF generation
ConclusionOur findings suggest that patients with acute ischaemic stroke have increased generation of MPs‐TF. Nevertheless, further studies are needed in order to confirm such inference.

Added Benefit of Stent Retriever Technology for Acute Ischemic Stroke: A Pooled Analysis of the NINDS tPA, SWIFT, and STAR Trials
CONCLUSION: Patients with acute ischemic stroke who received IV tPA or revascularization therapies had a higher likelihood of achieving independence at 3 months. Stent retriever technology combined with tPA was associated with the greatest benefit compared with placebo, tPA alone, or endovascular therapy alone. Telehealth for remote stroke management
This article reviews the rationale for telestroke networks and their current implementation in Canada. Telestroke networks enable stroke-specific procedures to be performed by less experiences physicians under the guidance of stroke neurology experts. The article also presents evidence that the safety and effectiveness of intravenous alteplase in community hospitals in a telestroke network seems to be comparable to that achieved in dedicated stroke centres. It is thus a viable option to guarantee an aging population access to stroke care across large geographic regions with faster treatment and access to more advanced trea...

A Paradigm Shift in EMS Evaluation Of Stroke
BackgroundMontgomery County Hospital District EMS (MCHD) is collaborating with the South East Texas Regional Advisory Committee (SETRAC) to validate their existing prehospital suspected neurological event guideline for stroke assessment and transport.MCHD is a non-fire based EMS service in Montgomery County, Texas. We respond to approximately 58,000 calls to service and identify stroke in more than 500 patients per year. Our EMS system covers an estimated 1,100-square-mile service area and has 190 medics, 900 EMTs and 15 first responder (FR) organizations.Acute ischemic stroke (AIS) is a debilitating and costly disease wit...

Efficency in Emergency Stroke Evaluation is Associated with Patient Volume in Louisianas Level III Hospitals (P6.268)
Conclusions:Higher volume Level III stroke hospitals are more efficient in the components of the stroke code resulting in shorter door-to-needle times for alteplase than lower volume hospitals. However, improvement in efficiency is needed. Global delays in resulting stroke labs may have contributed, but further research is necessary to identify the determinants of prolonged door-to-needle after completion of stroke code components.Disclosure: Dr. Martin-Schild has received personal compensation for activities with Genentech as a speaker.
