Intracranial hemorrhage in large vessel occlusion patients receiving endovascular thrombectomy with or without intravenous alteplase: a secondary analysis of the DIRECT-MT trial
Conclusions
Our data showed that in the DIRECT-MT trial, alteplase did not increase overall ICH for large vessel occlusion patients treated with thrombectomy, but it increased the parenchymal hematoma rate.

Hidden Telltale Signs in Hyperacute Ischemic Stroke Caused by Aortic Dissection: A Case Report and Post Hoc Analysis
Conclusion:
Hyperacute ischemic stroke caused by AD often hides some telltale signs. Clinicians should master basic clinical skills to exclude AD by looking for these telltale signs hidden in hyperacute ischemic stroke to avoid the fatal consequences of intravenous thrombolysis and/or cerebral angiography within the narrow window of time.

Cost-Effectiveness of Low-Dose Compared to Standard-Dose Alteplase for Acute Ischemic Stroke in China: A Within-Trial Economic Evaluation of the ENCHANTED Study
Conclusions: This health economic evaluation alongside the ENCHANTED indicates that the use of low-dose alteplase does not save overall healthcare costs nor lead to a gain in QALYs in the management of Chinese patients with AIS compared to the use of standard dose. Thrombolysis with alteplase 3 –4.5 hours after acute ischaemic stroke: trial reanalysis adjusted for baseline imbalances
In patients presenting to the emergency department with acute stroke, previous evidence from the Third European Cooperative Acute Stroke Study (ECASS III) recommended alteplase treatment within 3 to 4.5 hours of symptoms onset. Factors taken into consideration in the original report include time to treatment, history of smoking or hypertension, and an adjusted National Institute of Health Stroke Scale (NIHSS) score. However, it is unclear if accounting for history of stroke and baseline differences in NIHSS score on arrival may alter outcomes in patients given alteplase.

Sex-related differences among ischaemic stroke patients treated with intravenous thrombolysis in Poland.
CONCLUSIONS: Of the ischaemic stroke patients treated with intravenous thrombolysis, women had worse long-term outcomes than men. This discrepancy may be explained by the older age and higher proportion of cardio-embolic strokes with more severe baseline deficits. However, multiple logistic analysis did not show that sex itself had an impact on the greater mortality in women after a stroke, or on the poorer prognosis.

Current Smoking Does Not Modify the Treatment Effect of Intravenous Thrombolysis in Acute Ischemic Stroke Patients —A Post-hoc Analysis of the WAKE-UP Trial
Conclusions: In patients with mild to moderate MRI-proven AIS and unknown time of symptom onset with DWI-FLAIR mismatch, current smokers had worse functional outcome as compared to non-smokers.
